目的：探讨中心静脉置管术后低分子肝素预防血栓形成的临床意义。方法对该院收治的进行中心静脉置管术的恶性肿瘤患者按随机数字表分成治疗组和对照组。治疗组230例在中心静脉置管术后采用低分子肝素抗凝治疗，对照组230例不予抗凝治疗。结果治疗组发生血栓10例，对照组发生血栓26例，两组比较差异有统计学意义（P＜0.05）；在Caprini评分中极高危患者的血栓发生率较高危患者发生率高，但无统计学意义（P＞0.05）。结论恶性肿瘤患者中心静脉置管术后给予低分子肝素抗凝治疗，副反应轻，血栓的发生率明显下降，有临床推广意义。
목적：탐토중심정맥치관술후저분자간소예방혈전형성적림상의의。방법대해원수치적진행중심정맥치관술적악성종류환자안수궤수자표분성치료조화대조조。치료조230례재중심정맥치관술후채용저분자간소항응치료，대조조230례불여항응치료。결과치료조발생혈전10례，대조조발생혈전26례，량조비교차이유통계학의의（P＜0.05）；재Caprini평분중겁고위환자적혈전발생솔교고위환자발생솔고，단무통계학의의（P＞0.05）。결론악성종류환자중심정맥치관술후급여저분자간소항응치료，부반응경，혈전적발생솔명현하강，유림상추엄의의。
Objective To explore the clinical significance of low molecular heparin in the prevention of postoperative thrombosis after central venous catheter for malignant tumor.Methods 460 patients with malignant tumor who were admitted to our hospital and were indwelled central venous catheter were randomly divided into a treatment group and a control group according to the random number table.230 patients of the treatment group were given anticoagulant therapy with low molecular heparin after central venous catheter, while 230 patients of the control group were not given anticoagulant therapy.Results10 patients in the treatment group were founded thrombosis,while 26 patients in the control group were founded thrombosis.The difference between two groups was statistically significant(P<0.05),and had statistical significance;However,the Caprini score of the thrombosis incidence of ultra high-risk patients was relatively high compared with that of the high-risk patients,but the difference had no statistical significance (P>0.05).Conclusion Anticoagulation therapy with low molecular heparin after central venous catheter for malignant tumor has little side effects,can significantly decrease incidence of thrombosis and has clinical promotion significance.
