目的:探讨慢性阻塞性肺疾病(COPD)患者肺功能及呼吸困难程度与体质量指数(BMI)的相关性,为BMI用于COPD患者的诊断及病情评估提供参考依据。方法将156例COPD稳定期患者随机分为低BMI组、正常BMI组和高BMI组,分别进行肺功能检测和采用6MWT评估呼吸困难程度,分析BMI与肺功能及呼吸困难程度的相关性。结果(1)高低BMI两组FVC、FEV1%和FEV1/FVC均低于正常BMI组,三组FVC、FEV1%比较有统计学差异(P<0.05);而三组FEV1/FVC比较无统计学差异统计学(P>0.05)。(2)低BMI组和高BMI组在6min步行试验中有统计学差异(P<0.05),且低BMI组在重度和极重度中比例均明显高于高BMI组。结论 COPD患者体质量指数与肺功能及呼吸困难程度存在密切相关性,可为COPD患者的病情评估、后续辅助治疗及判断预后提供一定的参考依据。

Objective To explore the relationship between lung function, dyspnea and body mass index(BMI) in patients with chronic obstructive pulmonary disease(COPD),to provide evidence for diagnosis and assessment of the condition in patients with COPD with BMI.Methods156 patients were randomized into low BMI group, normal BMI group and high BMI group, the pulmonary function test was performed, and dyspnea was measured with 6MWT, the relationship between BMI,lung function and dyspnea were analyzed.Results (1)The FVC,FEV1 and FEV1/FVC of low BMI group and high BMI group were lower than normal BMI group, the FVC and FEV1 of three groups were significant difference (P<0.05); the FEV1/FVC of three groups were no significant difference (P>0.05). (2) The differences of 6MWT were significant difference between low BMI group and high BMI group(P<0.05), and the severe-to-very severe proportion of low BMI group was higher than high BMI group.Conclusion The relationship between lung function, dyspnea and BMI in patients with chronic obstructive pulmonary disease, which should provide the reference of assessment of the condition, evidence for diagnosis and prognosis in patients with COPD.
