目的：探讨治疗髋臼骨折合并臼顶区关节面塌陷的手术方法。方法对27例髋臼骨折合并臼顶区关节面塌陷患者采用手术切开复位重建钢板内固定治疗，对塌陷的关节面进行开窗复位和植骨手术。结果27例均获得随访，时间12~36个月。髋关节功能按改良的d′Aubigne-Postel 的标准评价：优15例，良9例，可2例，差1例，优良率24/27。结论髋臼骨折合并臼顶区关节面塌陷易漏诊，应加强对此类型骨折的认识；术中对塌陷部分进行开窗复位并植骨，可获得良好的临床效果。
목적：탐토치료관구골절합병구정구관절면탑함적수술방법。방법대27례관구골절합병구정구관절면탑함환자채용수술절개복위중건강판내고정치료，대탑함적관절면진행개창복위화식골수술。결과27례균획득수방，시간12~36개월。관관절공능안개량적d′Aubigne-Postel 적표준평개：우15례，량9례，가2례，차1례，우량솔24/27。결론관구골절합병구정구관절면탑함역루진，응가강대차류형골절적인식；술중대탑함부분진행개창복위병식골，가획득량호적림상효과。
Objective To explore the therapy for acetabular fracture with compressive articular surface of the acetabu-lar top. Methods Twenty-seven patients were treated by open reduction with internal fixation( ORIF) and the com-pressed bones were elevated with fenestration combined with bone grafts. All fractures were fixed with reconstruction plate. Results All patients were followed up from 12 months to 36 months. The result of modified d′Aubigne-Postel score system were excellent in 15 patients, good in 9, fair in 2 and poor in 1,the excellent-good rate was 24/27. Conclusions Patients of acetabular fracture combined with articular facet compression and collapse of acetabular top can be easily miss diagnosed. Understanding this type fracture should be strengthened to improve diagnosis. Good effect can be obtained with impacted bone elevation and bone graft.
