目的：探讨股骨近端抗旋髓内钉( proximal femoral nail antirotation ，PFNA )治疗老年不稳定性股骨粗隆间骨折的临床疗效。方法2011-10—2014-02收治老年不稳定性股骨粗隆间骨折患者47例，均应用PFNA行内固定手术治疗，评价其临床效果。结果所有病例均获得随访，随访时间6～18个月，平均13个月。手术时间25～73 min ，平均42.4 min。术中出血量40～150 mL ，平均70.2 mL。所有病例均获得骨性愈合，且未出现内固定松动、断裂等并发症。疗效依照Harris标准评估，优29例，良15例，可3例，优良率93.6%。结论 PFNA具有髓内固定的生物学优势，且创伤小、操作简单、固定牢靠，是治疗老年不稳定性股骨粗隆间骨折的一种安全可靠的治疗方法。

Objective To investigate the clinical effect of proximal femoral nail anti-rotation(PFN A ) to treat unstable femoral intertrochanteric fractures of the elderly.Methods 47 patients with unstable femoral intertrochanteric fractures in the elderly admitted fromO ctober 2011 and February 2014 were all treated with internal fixation operations by means of PFN A .Results A follow-up visit of 6~18 months was done in all of the 47 patients with an average of 13 months.The operation time was 25~73 minutes with an average 42 .4 minutes.B lood loss during operation was 40~150 mL with an average of 70 .2 mL .All patients got bone union ,and did not showcomplications such as internal fixa-tion loosening or fracture.According to Harris standard assessment,the curative effect showed 29 excellent cases,15 good cases,and 3 acceptable cases,with an excellent and good rate of 93.6% .Conclusion PFN A bears the biological advantages of intramedullary fixation,as well as minimal trauma,simple operation,and stable fixation,which is a safe and reliable treatment for unstable femoral intertrochanteric fracture in the elderly.
