高龄者医疗保险制度是日本医疗保险制度的重要组成部分，该制度以在职劳动者缴费和财政的转移支付为主要资金来源，实质是为高龄老人提供医疗福利。该制度设立之初便埋下隐患。随着老龄化和少子化形势的日益严峻，这一制度使在职劳动者社会负担不断加重，医疗保险财政陷入困境。日本政府为了赢得国民信赖，建立可持续的医疗保险制度，对高龄者医疗保险制度进行改革。但是，急速的高龄少子化、经济的低速运行、国民健康意识的增强，使日本医疗保险改革的外部条件不容乐观。这对中国医疗保险制度改革具有启示意义。

The medical insurance system for persons of advanced age, as an important part of Japan＇s medical insurance system, takes payments by workers in office and financial transfer payments as main fund. The system is essentially a kind of medical welfare for persons of advanced age and had hidden trouble at the beginning of establishment of the system. With the aging of population becoming increasingly severe and the rate of birth falling, the system has been a heavy social burden to workers in office, and medical insurance finance has been in a difficult position. Japanese Government reforms medical insurance system for persons of advanced age to win national trust and establish sustainable medical insurance system. However, Japan＇s medical insurance reform is not optimistic at all because of rapid aging of population, falling rate of birth, economic development at low speed, and national health consciousness enhancement. This is of significance to China＇s medical insurance system reform.
