目的:观察常规训练中非力竭耐力运动前后少年运动员血清心肌损伤标志物水平的变化。方法:测定19名少年男子中长跑运动员在常规训练中一次非全力10 km跑前、运动后2 h、运动后4 h以及运动后24 h血清心肌肌钙蛋白T亚基(cTnT)、I亚基(cTnI)、CK-MB活性及CK-MB质量水平。结果:10 km跑可显著提高被试者血清cTnT、cTnI及CK-MB质量水平。运动后4 h内血清cTnT和cTnI可超过临床心肌损伤(60%以上被试者)甚至心肌梗死(10.5%~42.1%被试者)界值,而运动后24 h,血清cTnT及cTnI恢复至正常水平。另外,运动前后各观察点都有相当比例的被试者(15.8%~84.2%)CK-MB活性及质量超过心肌损伤界值。结论:非力竭性耐力运动可能引起少年运动员可逆性心肌微损伤,但其确切机制、临床意义及远期后果有待进一步研究。

목적:관찰상규훈련중비력갈내력운동전후소년운동원혈청심기손상표지물수평적변화。방법:측정19명소년남자중장포운동원재상규훈련중일차비전력10 km포전、운동후2 h、운동후4 h이급운동후24 h혈청심기기개단백T아기(cTnT)、I아기(cTnI)、CK-MB활성급CK-MB질량수평。결과:10 km포가현저제고피시자혈청cTnT、cTnI급CK-MB질량수평。운동후4 h내혈청cTnT화cTnI가초과림상심기손상(60%이상피시자)심지심기경사(10.5%~42.1%피시자)계치,이운동후24 h,혈청cTnT급cTnI회복지정상수평。령외,운동전후각관찰점도유상당비례적피시자(15.8%~84.2%)CK-MB활성급질량초과심기손상계치。결론:비력갈성내력운동가능인기소년운동원가역성심기미손상,단기학절궤제、림상의의급원기후과유대진일보연구。
Purpose: This study was designed to examine the appearance and clearance characteristics of serum cardiac markers in junior runners after a non-exhaustive endurance exercise.Method:19 trained male adolescents performed non-all-out 10-km run during routine training.The levels of cTnT,cTnI and CK-MB mass,and activities of CK-MB immediately before and two,four and twenty-four hours after exercise were measured.Results: At pre-exercise,cTnT and cTnI were at the normal range.However,CK-MB activity and CK-MB mass were above corresponding clinical cut-off values for myocardial injury.Post-exercise significant elevations above pre-exercise were observed for all biomarkers,except CKMB activity,two and four hours following the run.Similar changes were observed in the biomarkers positive rate in the subjects.Conclusions: Such findings suggest that non-exhaustive long-distance run during routine training might lead to reversible minor myocardial injury in junior runners.However,its clinical significance,long-term consequences and mechanism remain to be fully elucidated.
