目的:研究肥胖儿童少年血内脂素水平及其与身体形态、体脂分布状况、糖代谢与脂代谢参数及瘦素的关系,探讨在儿童少年肥胖过程中内脂素对糖、脂代谢的可能调节机制。方法:39名肥胖儿童少年(BMI 28.98±4.64kg/m2)测血清内脂素和瘦素水平,并测定血脂6项、胰岛素、血糖、糖化血红蛋白(HBA1C)等糖、脂代谢指标和体脂率(F%)、腰臀比(WHR)等形态学指标。采用Pearson相关分析、偏相关分析和独立样本t检验对各参数间关系进行检验。结果:1)高visfatin组HDL、leptin水平显著高于低visfatin组(P≤0.05);2)血visfatin与leptin水平高度相关(r=1,P≤0.01);3)血visfatin与糖代谢指标无相关,但与HDL显著相关(r=0.313,P≤0.05),以年龄、性别和BMI作为控制变量偏相关分析后相关性仍存显著性(r=0.344,P≤0.05)。结论:所测肥胖儿童少年血内脂素与瘦素高度相关,体内尤其是脂肪组织中瘦素与内脂素的产生可能存在着非常紧密的关联。内脂素参与了体内胆固醇代谢,可能是联系NAD和HDL代谢过程的结合点。

목적:연구비반인동소년혈내지소수평급기여신체형태、체지분포상황、당대사여지대사삼수급수소적관계,탐토재인동소년비반과정중내지소대당、지대사적가능조절궤제。방법:39명비반인동소년(BMI 28.98±4.64kg/m2)측혈청내지소화수소수평,병측정혈지6항、이도소、혈당、당화혈홍단백(HBA1C)등당、지대사지표화체지솔(F%)、요둔비(WHR)등형태학지표。채용Pearson상관분석、편상관분석화독립양본t검험대각삼수간관계진행검험。결과:1)고visfatin조HDL、leptin수평현저고우저visfatin조(P≤0.05);2)혈visfatin여leptin수평고도상관(r=1,P≤0.01);3)혈visfatin여당대사지표무상관,단여HDL현저상관(r=0.313,P≤0.05),이년령、성별화BMI작위공제변량편상관분석후상관성잉존현저성(r=0.344,P≤0.05)。결론:소측비반인동소년혈내지소여수소고도상관,체내우기시지방조직중수소여내지소적산생가능존재착비상긴밀적관련。내지소삼여료체내담고순대사,가능시련계NAD화HDL대사과정적결합점。
Objective:To assess visfatin(PBEF/Nampt) levels and the relationships between concentrations of visfatin as a novel adipocytokine and morphological characters,glucose and lipid metabolism,lepin levels in obese children and adolescents and moreover to explore the regulation mechanism of recently described adipokine on obesity.Methods:We measured serum visfatin and leptin concentrations of 39 subjects with BMI 28.98±4.64 kg/m2.In addition,glucose and lipid metabolism parameters and morphological characters were assessed.Pearson correlation coefficients,partial correlation coefficients and independent-samples t test were performed to detect associations of the parameters.Results:1) HDL and leptin concentrations of high-visfatin group were significantly higher than those of low-visfatin group(P≤0.05).2)A strong correlation was observed between serum visfatin and leptin(r=1,P≤0.01).3)Visfatin was significantly positively correlated with HDL by pearson correlation coefficients(r=0.313,P≤0.05)and partial correlation coefficients(r=0.344,P≤0.05),while no significant correlation with glucose metabolism.Conclusion:The data indicates that a strong correlation exists between visfatin and leptin to achieve the regulation of glucose and lipid metabolism in obese children and adolescent.Involved in cholesterol metabolism,visfatin could be the binding site between metabolism of NAD and HDL.
