在社会市场经济制度下,以社会福利原则为基础的社会医疗保险制度是德国医疗制度的核心和保证,互助原则是其鲜明的特质。进入20世纪90年代之后,德国的医疗制度开始引入竞争因素和竞争机制,希望能够在保证社会公平的同时提供更有效率的医疗服务,为传统医疗制度补充新的活力。互助原则与竞争机制在医疗制度改革的过程中究竟是一种怎样的关系？德国的医疗制度改革实践又是如何处理二者之间的相互关系的？文章从德国医疗制度的社会福利化特征、改革的路径选择及兼容性等方面进行了梳理。

The German medical system under the social market economy has unique characteristics, with a social health insurance system based on the principle of social welfare at its core, and the principle of reciprocity as a distinctive characteristic. In the 1990s, competition was introduced into the German medical system, in the hope of providing more efficient medical services and injecting new vitality to the traditional medical system, while maintaining social equality and justice. What is the relationship between reciprocity and competition in the process of medical system reform？ How was this relationship dealt with during the reform of Germany＇s medical system？ This paper discusses theoretically the historical development of Germany＇s medical system in terms of its social welfare characteristics, the reform path chosen and its compatibility.
