目的:采用全脑功能磁共振成像(1H-MRS)方法分析有氧运动综合干预前、后肥胖儿童额叶、海马感兴趣区脑代谢的变化特点,以事件相关电位P300潜伏期反映肥胖儿童注意力的分布、执行功能和学习记忆等情况,探讨有氧运动综合干预对肥胖儿童额叶和海马感兴趣区脑代谢及认知功能的影响。方法:对象为2012-2013年北京市金色湖畔儿童夏令营的小学生。采用有氧运动综合干预措施对11名肥胖儿童进行饮食、运动、教育联合干预治疗,应用多维度神经心理测试对比评估肥胖儿童干预前、后的认知功能,磁共振测定脑部海马和额叶部位脑代谢的变化特点,时间相关电位测定P300潜伏期。结果:有氧运动综合干预后,肥胖儿童神经心理学测验各项指标学习记忆能力、注意能力、执行功能以及语言能力均显著升高(P〈0.01);额叶部位的氮-乙酰天门冬氨酸(NAA)、额叶胆碱类复合物(Cho)浓度、颞叶氮-乙酰天门冬氨酸与肌酸比值(NAA/Cr)均较干预前显著升高(P〈0.05);P300潜伏期显著缩短。结论:肥胖可能影响儿童的脑代谢过程,并对认知功能产生负性影响;有氧运动综合干预对于提高肥胖儿童的认知功能具有统计学上的显著性意义,有氧运动综合干预对肥胖儿童认知功能的改善主要是通过提高肥胖儿童额叶感兴趣区脑代谢来实现的。

Objective:To study the change characteristics of aerobic exercise in obese children before and after comprehensive intervention of frontal lobe and hippocampus region of the brain in the sense of metabolism. Take the event-related potential P300 latency to reflect distribution of obese children with attention, executive function and learning memory, this paper analyzes the effect of comprehensive intervention of aerobic exercise on the frontal lobe and hippocampus of sense in brain metabolism and cognitive function of obese children. Methods:the subject is the primary students from Beijng Golden Lake children's summer camp in 2012--2013, with aerobic exercise comprehensive intervention measures on 11 obese children of diet, exercise, education combined intervention therapy, using multidimensional neuropsychologieal tests and com- parative assessment of cognitive function hefore and after the intervention of childhood obesity, the determination of brain hippocampus and frontal lobe of brain metabolic changes of magnetic resonance, event-related potential determination of P300 latency. Result:After aerobic exercise intervention, obese children neuropsychological test indicators of learning memory ability, attention and executive function and language ability were significantly higher (P 〈 0.0 1 ) ; frontal lobe of N-acetyl aspartic acid (NAA), frontal lobe choline compound (CHO) concentration, temporal leaf N-aeetyl aspartic acid and creatine ratio (NAA/Cr) were compared with those before intervention was significantly increased (P〈0.0 5 ) ;P300 latency significantly shortened. Conclusion:Obesity may affect children's brain metabolic processes, and have a negative impact on cognitive function, aerobic exercise intervention to improve cognitive function in obese children with statistical significance, aerobic exercise intervention on cognitive function in children with obesity improvement is mainly through the increase of the frontal lobe of obese children of cerebral metabolism.
