目的:探讨三甲医院护理人力资源配置的最佳比例,为人力资源配置标准提供依据,指导临床护理人员的合理调配. 方法:以咸宁市某三甲医院为研究现场,选取创伤骨科、口腔科、神经外科等范围内共25个临床科室作为研究对象,在卫生部测算标准的基础上,结合咸宁市某三级甲等医院的实际情况进行增或减直接护理及间接护理项目,通过前瞻性设计实时记录护理工作量方式测算护理工作量,采取大样本、长时间、综合各种类别科室测量,分析描述护理工作类型以及工作量的时间分布、空间分布特征. 结果:全院各科室的床位使用率≥100%,12个科室护士配置人数较合理,其中2个监护病区最为到位;直接护理时间占总护理时间较小的有12个科室,直接护理时间占总护理时间较大的科室有13个. 结论:该院部分科室护理人力资源配置存在短缺问题,护士配置人数较少、质量较低,护理人员直接护理时间过长,有待于改善.

Objective:To explore the optimal proportion in the allocation of nursing human resources in the hospital at the level of grade Ⅲ and class A in order to provide the basis for the standard allocation of nursing human resources. Methods:25 clinical departments in a certain hospital at the level of grade Ⅲ and class A in Xianning city were taken as the object of study. On the basis of the standard of the Ministry of Health,we increased and decreased nursing items combining with the actual situation of the hospital and measured the nursing workload through the real-time recording of prospective design. Results:The bed utilization rate in all the departments of the hospital was more than or equal to 100%,the reasonable allocation of nurses was found in 12 departments. The direct nursing time was less in the total nursing time in 12 departments and much in 13 departments. Conclusion:The hospital has the shortage of nursing human resources and the direct nursing time is longer.
