目的 系统评价国内使用氯己定口腔护理预防呼吸机相关性肺炎的有效性. 方法 计算机联机检索CNKI、VIP、万方数据库(各数据库检索时间为创建至2014年12月)中关于使用氯己定口腔护理预防呼吸机相关性肺炎的随机对照试验( RCT) ,用RevMan5. 3软件对数据进行Meta分析. 结果 共纳入15篇RCT,包括1909例患者. Meta分析结果显示:使用氯己定口腔护理预防呼吸机相关性肺炎的效果优于对照组[RR=0. 45,95%CI(0. 38,0. 54),P<0. 01].结论 基于现有临床证据,使用氯己定口腔护理预防呼吸机相关性肺炎的有效性较高. 但由于纳入研究质量较差,本结论尚需要更高质量临床RCT予以证实.

Objective To systematically evaluate the efficacy and safety of oral care with chlorhexidine in preventing ventilator associated pneumonia in Chinese patients. Methods Studies were identified through CNKI,VIP and WANFANG Data-bases. Eligible studies were randomized controlled trials(RCT)of oral care with chlorhexidine in preventing ventilator associated pneumonia among Chinese patients. Meta analysis was carried out by RevMan 5. 3 software. Results 15 RCTs were included (1909 patients). The Meta analyses showed that oral care with chlorhexidine in preventing ventilator associated pneumonia were superior to those of the control group[RR=0. 45,95%CI(0. 38,0. 54), P<0. 01]. Conclusion The present meta-analysis in-dicates a beneficial effect of oral care with chlorhexidine in preventing ventilator associated pneumonia. However, further studies with rigorous design and large sample size are needed in the future because of study heteogeneity.
