根治性膀胱全切术是治疗肌层侵袭性膀胱癌的标准方法 ,理想的外科治疗是既能彻底切除肿瘤 ,又能最大限度地保留机体功能.腹腔镜辅助下保留性神经的根治性膀胱全切和原位新膀胱重建为实现上述目的提供了有力的技术支撑.本文探讨了腹腔镜根治性膀胱全切中盆腔淋巴结清扫、性神经的保留、尿控的保护、尿道和新膀胱的重建等手术的关键技巧问题.

Radical cystectomy is a surgical standard in the treatment of muscle invasive bladder cancer .The ideal purpose is that the bladder cancer is resected completely and physical function is obtained meanwhile .Laparoscopic radical cystectomy with nerve sparing and neobladder construction becomes a confirmed approach .In this paper ,the author introduced the critical surgical issues including the adequacy of pelvic lymph node dissection (PLND) ,nerve sparing ,continence protection and ure-thral-neobladder reconstruction based on his surgical experience .
