目的 探讨大黄川芎汤对严重脓毒症患者血乳酸及乳酸清除率的影响.方法 按照随机数字表的分组方法将90例严重脓毒症患者分成观察组和对照组,每组45例.对照组患者接受脓毒症集束化治疗,观察组在对照组的基础上给予大黄川芎汤鼻饲. 比较两组患者治疗后6 h、24 h及48 h的APCHEⅡ评分及治疗后6 h及24 h的血乳酸水平, 计算6 h及24 h乳酸清除率. 结果 观察组治疗后6 h、24 h及48 h的APCHEⅡ评分均低于对照组,两组比较差异均有统计学意义(P<0.05). 观察组治疗后6 h、24 h血乳酸水平及乳酸清除率均优于对照组,两组比较差异均有统计学意义(P<0.05). 结论 大黄川芎汤可改善严重脓毒症患者急性生理紊乱状况,能有效降低患者血乳酸水平及乳酸清除率,对严重脓毒症患者具有较好的临床应用价值.

목적 탐토대황천궁탕대엄중농독증환자혈유산급유산청제솔적영향.방법 안조수궤수자표적분조방법장90례엄중농독증환자분성관찰조화대조조,매조45례.대조조환자접수농독증집속화치료,관찰조재대조조적기출상급여대황천궁탕비사. 비교량조환자치료후6 h、24 h급48 h적APCHEⅡ평분급치료후6 h급24 h적혈유산수평, 계산6 h급24 h유산청제솔. 결과 관찰조치료후6 h、24 h급48 h적APCHEⅡ평분균저우대조조,량조비교차이균유통계학의의(P<0.05). 관찰조치료후6 h、24 h혈유산수평급유산청제솔균우우대조조,량조비교차이균유통계학의의(P<0.05). 결론 대황천궁탕가개선엄중농독증환자급성생리문란상황,능유효강저환자혈유산수평급유산청제솔,대엄중농독증환자구유교호적림상응용개치.
Objective To investigate the effects of Dahuang Chuanxiong decoction on blood lactate and lactate clearance in patients with severe sepsis. Methods According to the grouping method of random number table,90 cases of severe sepsis patients were divided into observation group and control group,45 cases in each group. The control group were received sepsis bundle therapy,the observation group were received nasal feeding Dahuang Chuanxiong decoction. The APCHE score at 6 h,24 h and 48 h and the blood lactic acid level and lactate clearance rate after 6 h and 24 h treatment of the two groups were compared. Results The APCHE score at 6 h,24 h and 48 h in the observation group were all better than those in the control group(P<0.05). The blood lactic acid level and lactate clearance rate after 6 h and 24 h treatment in the observation group were all better than those in the control group (P<0.05). Conclusion Dahuang Chuanxiong decoction could improve acute physiology disorder in patients with severe sepsis,can effectively reduce the blood lactic acid level and lactate clearance rate,which has better clinical value for patients with severe sepsis.
