目的 调查了解某部装甲步兵在长时间谐振环境下椎间盘源性下腰痛发生情况及腰背肌相对肌力训练后对其改善情况. 方法 采用自制调查表及拟定椎间盘源性下腰痛诊断标准,对某部402名装甲步兵在谐振环境下椎间盘源性下腰痛发生情况进行调查, 比较不同专业和军龄椎间盘源性下腰痛人员的发生率及疼痛程度,并对椎间盘源性下腰痛装甲步兵采取腰背肌锻炼方法,明确其改善情况. 结果 共发生椎间盘源性下腰痛37例,发生率9.2%.其中乘员和载员分别为27例、10例,分别占73.0%、27.0%,两组比较有显著差异(P<0.05).1~2年和≥2年军龄组椎间盘源性下腰痛发生率显著高于≤1年军龄组 (P<0.05),≤1年和1~2年军龄组相比差异显著(P>0.05).Ⅰ度疼痛共发生25例,Ⅱ度疼痛发生12例,Ⅰ度疼痛明显高于Ⅱ度疼痛(P<0.05).通过8周的腰背肌锻炼后,载员的有效率明显高于乘员(P<0.05);锻炼前与锻炼后比较,有显著性差异(P<0.05).结论 某部装甲步兵椎间盘源性下腰痛发生率较高,乘员发生率高于载员,≥2年军龄发生率最高,疼痛程度多为Ⅰ、Ⅱ度,通过适当的腰背肌相对肌力训练,对Ⅰ、Ⅱ度疼痛的缓解有效,可作为治疗并预防Ⅰ、Ⅱ度椎间盘源性下腰痛患者的常规措施.

Objective To investigate the incidence of intervertebral disk-derived low back pain caused by long-last syntony situation among armored infantry and the improvement of low back muscle after relative strength exercise. Methods With the forms and the diagnostic standard of intervertebral disk-derived low back pain , authors conducted a investigation of 402 armored infantry in long-last syntony situation and compared the incidence of intervertebral disk-derived low back pain and the degree of pain. By teaching relative strength exercise of low back muscle,authors expected to improve the severity of their low back pain. Results There were 37 cases of intervertebral disk-derived low back pain patients among the 402 armored infantry accounted for 9.2%. Among them,27 were the staff working on vehicles with accounted for 73.0% and 10 were the carried passengers with accounted for 27.0%. And there was significant difference between both (P<0.05). The incidence of intervertebral disk-derived low back pain in 1-2 years and ≥2 years of military service group was significantly higher than that ≤1 year of military service group and the differences were statistically significanct (P<0.05);25 cases were in Ⅰ degree pain which was much higher than the ones with Ⅱ degree pain in 12 cases (P<0.05). After eight weeks of relative strength exercise of low back muscle ,the effective rate was better in the carried passengers than that in staffworking on vehicles (P<0.05). Between before and after the exercise,there was statistically significant difference (P<0.05). Conclusions There has a high incidence rate of intervertebral disk-derived low back pain in armored infantry. It is higher in staff working on vehicles than the carried passengers. ≥2 years of military service group has the highest incidence. Ⅰ and Ⅱ pain degree occured predo-minantly. The relative strength exercise of low back muscle ,could improve the pain and prevent the intervertebral disk-derived low back pain of Ⅰ and Ⅱ degree severity.
