目的:探讨呼吸机无创机械通气治疗老年慢性阻塞性肺疾病( COPD )合并呼吸衰竭的临床效果。方法选择COPD合并呼吸衰竭患者100例,随机分为两组,均采用常规抗炎、平喘治疗等基础治疗,对照组给予持续低流量吸氧治疗,观察组给予无创机械通气,比较两组治疗前及治疗24h后血气分析、心率及呼吸频率变化,比较治疗3d后有效率。结果两组治疗后血气分析指标、呼吸频率及心率均比治疗前有所改善( P <0.05),治疗后,观察组PaO2高于对照组( P <0.01),PaCO2、心率及呼吸频率低于对照组( P <0.01)。治疗3d后总有效率84.00%高于对照组48.00%( P <0.01)。结论呼吸机无创机械通气治疗老年COPD合并呼吸衰竭患者可在24h内改善血气分析指标,降低心率及呼吸频率,提高治疗效果,值得临床推广。

목적:탐토호흡궤무창궤계통기치료노년만성조새성폐질병( COPD )합병호흡쇠갈적림상효과。방법선택COPD합병호흡쇠갈환자100례,수궤분위량조,균채용상규항염、평천치료등기출치료,대조조급여지속저류량흡양치료,관찰조급여무창궤계통기,비교량조치료전급치료24h후혈기분석、심솔급호흡빈솔변화,비교치료3d후유효솔。결과량조치료후혈기분석지표、호흡빈솔급심솔균비치료전유소개선( P <0.05),치료후,관찰조PaO2고우대조조( P <0.01),PaCO2、심솔급호흡빈솔저우대조조( P <0.01)。치료3d후총유효솔84.00%고우대조조48.00%( P <0.01)。결론호흡궤무창궤계통기치료노년COPD합병호흡쇠갈환자가재24h내개선혈기분석지표,강저심솔급호흡빈솔,제고치료효과,치득림상추엄。
Objective To investigate clinical efficacy of noninvasive mechanical ventilation in the treatment of elderly patients with chronic obstructive pulmonary disease ( COPD) complicated with respiratory failure.Methods One hundred patients suffering from COPD with respiratory failure were randomly divided into 2 groups, undergoing basic anti-inflammatory and anti-asthmatic treat-ment.The control group with continuous low flow oxygen therapy and the observation group in the treatment of noninvasive mechanical ventilation, both of them were observed before and after treatment.Results The two groups were analyzed by arterial blood gas analy-sis,respiratory and heart rate.All patient’ s conditions were improved after treatment compared with those before treatment( P <0. 05).PaO2 of the observation group after treatment was higher than the of control group(Ρ<0.01), while PaCO2,heart rate and res-piration rate of the observation group were lower than those of the control group( P <0.01).The total effective rate of the observation group was 84%higher than that of the control group after 3d (Ρ<0.01).Conclusion Noninvasive mechanical ventilation in the treatment of elderly patients with COPD complicated with respiratory failure can improve the index of blood gas analysis and reduce heart rate and respiration rate after 24h.It can improve the therapeutic effect.
