目的 探讨低剂量CT在非小细胞肺患者围术期诊治中的应用价值. 方法 选择80例经穿刺或纤支镜组织病理学检查证实的非小细胞肺癌患者,按照随机数字表法分为实验组和对照组,每组40例. 实验组患者术前、术后行低剂量CT扫描,对照组患者行常规剂量CT扫描. 分别对两组患者的术前肺癌征象的显示状况、图像质量以及剂量、术后并发症等结果进行分析比较. 结果 实验组患者经低剂量CT扫描后,显示具有钙化、坏死、细短毛刺、分叶、磨玻璃、空泡、血管集束及胸膜凹陷等肺癌征象分别有8例、10例、21例、18例、5例、4例、13例与14例,与对照组患者比较,其差异无统计学意义( P>0.05);实验组患者经低剂量CT扫描后,37例图像质量为优,3例图像为良,0例图像为差;对照组患者31例图像质量为优,6例图像为良,3例图像为差. 两组患者的CT图像的质量具有明显差异性(P<0.05).实验组患者的CTDIvol的范围为(10.135 ±1.453)mGy,DLP的范围为(98.125 ±15.515)mGy/cm,均显著低于对照组患者的CTDIvol与DLP剂量,具有显著差异性(P<0.05);实验组患者术后经低剂量CT扫描后,显示患有肺炎、胸腔积液、气胸、骨断裂、软组织损伤的例数分别有13例、11例、8例、5例、7例与3例,与对照组患者比较,其差异无统计学意义( P>0.05). 结论 低剂量CT扫描能够显示较为清晰的钙化、坏死、细短毛刺等术前肺癌征象,对非小细胞肺癌的诊断及术后评价有重要价值.

목적 탐토저제량CT재비소세포폐환자위술기진치중적응용개치. 방법 선택80례경천자혹섬지경조직병이학검사증실적비소세포폐암환자,안조수궤수자표법분위실험조화대조조,매조40례. 실험조환자술전、술후행저제량CT소묘,대조조환자행상규제량CT소묘. 분별대량조환자적술전폐암정상적현시상황、도상질량이급제량、술후병발증등결과진행분석비교. 결과 실험조환자경저제량CT소묘후,현시구유개화、배사、세단모자、분협、마파리、공포、혈관집속급흉막요함등폐암정상분별유8례、10례、21례、18례、5례、4례、13례여14례,여대조조환자비교,기차이무통계학의의( P>0.05);실험조환자경저제량CT소묘후,37례도상질량위우,3례도상위량,0례도상위차;대조조환자31례도상질량위우,6례도상위량,3례도상위차. 량조환자적CT도상적질량구유명현차이성(P<0.05).실험조환자적CTDIvol적범위위(10.135 ±1.453)mGy,DLP적범위위(98.125 ±15.515)mGy/cm,균현저저우대조조환자적CTDIvol여DLP제량,구유현저차이성(P<0.05);실험조환자술후경저제량CT소묘후,현시환유폐염、흉강적액、기흉、골단렬、연조직손상적례수분별유13례、11례、8례、5례、7례여3례,여대조조환자비교,기차이무통계학의의( P>0.05). 결론 저제량CT소묘능구현시교위청석적개화、배사、세단모자등술전폐암정상,대비소세포폐암적진단급술후평개유중요개치.
Objective To explore the application value of low-dose CT in the diagnosis and treatment of perioperative non-small cell lung cancer.Methods 80 patients received biopsy or bronchoscopy and confirmed by histopathology to be non-small cell lung cancer,according to a random number table,were divided into the experimental group and the control group,40 pa-tients in each group.The experimental group received preoperative and postoperative low-dose CT scan,the control group received routine-dose CT scan.Preoperative condition of lung cancer signs,image quality and dose,postoperative complications of the 2 groups were analyzed and compared.Results The experimental group received low-dose CT scan,the result showed that calcifi-cation,necrosis,short burr,leaf,ground glass,vacuoles,vascular bundle of pleural indentation signs were 8 cases,10 cases,21 ca-ses,18 cases,5 cases,4 cases,13 cases and 14 cases,and compared with the control group,no patients,the difference was not sta-tistically significant (P>0.05);the experimental group received low-dose CT scan,37 cases of image quality was excellent,3 ca-ses of image was good,0 case the image was poor;in the control group,31 patients had excellent image quality,6 cases had benign image,3 cases had poor image.The quality of the 2 groups with CT images had significant difference (P<0.05).Scope of the ex-perimental group of CTDIvol was (10.135 ±1.453) mGy,the range of DLP was (98.125 ±15.515) mGy /cm,which were significantly lower than CTDIvol and DLP dose of the control group,there had significant difference (P<0.05 );the experimental group after low-dose CT scans showed that pneumonia,pleural effusion,pneumothorax,bone fracture,the number of patients with soft tissue injury were 13 cases,11 cases,8 cases,5 cases,7 cases and 3 cases,compared with the control group,there had no sig-nificant difference (P>0.05).Conclusion Low-dose CT scan can show clear calcification,necrosis,small burrs and other pre-operative lung cancer signs,it has important value in the diagnosis and postoperative evaluation of non-small cell lung cancer.
