目的::利用IgA肾病大鼠模型研究活血养阴方的疗效及其作用机制。方法:将Sprague-Dawley( SD)大鼠随机分为正常对照组、模型组、缬沙坦组、中药组,每组12只。采用复合免疫法建立IgA肾病大鼠模型,于造膜成功后第10周起分别按各组相应药物剂量灌胃。第15周末检测24 h尿蛋白定量( UP)、血肌酐( Scr)、尿素氮( BUN)及IgA免疫荧光检测,光镜下观察大鼠肾脏病理学改变。结果:中药组、缬沙坦组大鼠与模型组大鼠24 h UP、Scr、BUN比较差异有统计学意义(P<0.01),中药组效果明显(P<0.05);与模型组比较,各治疗组IgA免疫荧光强度显著下降(P<0.05),肾组织病理学改变得到一定缓解(P<0.05),其中中药组在系膜增生及肾血管病变方面疗效更佳(P<0.05)。结论:活血养阴方能减少IgA肾病大鼠蛋白尿,保护肾功能,其作用机制可能与减少异常糖基化IgA1在系膜区的沉积,抑制肾小球系膜细胞增殖及细胞外基质积聚,改善肾脏微循环有关。

목적::이용IgA신병대서모형연구활혈양음방적료효급기작용궤제。방법:장Sprague-Dawley( SD)대서수궤분위정상대조조、모형조、힐사탄조、중약조,매조12지。채용복합면역법건립IgA신병대서모형,우조막성공후제10주기분별안각조상응약물제량관위。제15주말검측24 h뇨단백정량( UP)、혈기항( Scr)、뇨소담( BUN)급IgA면역형광검측,광경하관찰대서신장병이학개변。결과:중약조、힐사탄조대서여모형조대서24 h UP、Scr、BUN비교차이유통계학의의(P<0.01),중약조효과명현(P<0.05);여모형조비교,각치료조IgA면역형광강도현저하강(P<0.05),신조직병이학개변득도일정완해(P<0.05),기중중약조재계막증생급신혈관병변방면료효경가(P<0.05)。결론:활혈양음방능감소IgA신병대서단백뇨,보호신공능,기작용궤제가능여감소이상당기화IgA1재계막구적침적,억제신소구계막세포증식급세포외기질적취,개선신장미순배유관。
Objective:To explore the effect and acting mechanism of Huoxue Yangyin Fang on treating IgA nephropathy. Methods:48 SD rats were randomly divided into 4 groups:control group, model group, valsartan group and Chinese medicine group. The IgA nephropathy rat model was induced by multiple immune. From the tenth week, rats were afforded with the medicine respec-tively. At the fifteenth week, urinary protein, serum levels of creatinine ( Scr) , blood ureanitrogen ( BUN) and IgA immunofiuores-cence were detected with relative technique. Renal pathological changes were observed by electron microscopy. Results:In contrast with model group, the level of 24 h urine protein quant, Scr and BUN were significantly decreased in valsartan and Chinese medicine group (P<0. 01), while the Chinese medicine group was better than valsartan group (P<0. 05). In contrast with model group, the level of IgA immunofiuorescence were significantly decreased (P<0. 05) and the histopathological changes of the kidneys were re-lieved in each treating group (P<0. 05), especially the mesangial cell proliferation and renal vascular lesions in Chinese medicine goup (P<0. 05). Conclusion:Huoxue Yangyin Fang can reduce proteinuria, improve renal function and reduce the pathological changes. The mechanism may be related to reducing the Abnormal Glycosylation of IgA1 deposition in renal tissue , inhibiting me-sangial cell proliferation and extracellular matrix accumulation and improving the renal microcirculation.
