目的 探讨CT引导下穿刺活检表皮生长因子受体表达对于肺癌的诊断价值. 方法 选择行穿刺活检的肺癌患者72例,根据随机抽签原则分为2组,各36例. B超组采用B超引导下穿刺活检,CT组采用CT引导下穿刺活检. 结果 CT组的一次性穿刺成功为100.0%,B超组为86.1%,差异有统计学意义(P<0.05). CT组的肺癌检出率为91.7%,而B超组的肺癌检出率为86.1%,差异无统计学意义(P>0.05). 表皮生长因子受体在CT组中的阳性表达率为97 .2%,在B超组中的阳性表达率为83 .3%,组间对比差异有统计学意义( P<0 .05 ). 穿刺过程中CT组的气胸、痰血等并发症发生率明显少于B超组,对比差异明显(P<0.05). 结论 CT引导下穿刺活检在肺癌中的诊断有很好的定位价值,可以提高穿刺活检的准确性,有利于表皮生长因子受体的表达检测,且能够减少并发症的发生.

Objective To investigate the diagnostic value of CT-guided needle biopsy of the epidermal growth factor re-ceptor expression in lung cancer.Methods 72 lung cancer patients were equally divided into the CT group and the ultrasound group(group B).Group B received ultrasound-guided biopsy,and the CT group received CT-guided biopsy.Results The dispos-able puncture success rates in the CT group and the group B were 100.0%and 86.1%,there had statistically significant differ-ence (P<0.05).The lung cancer detection rate in the CT group was 91.7%,while that of the group B was 86.1%,there had no significant difference (P>0.05).The positive expression of epidermal growth factor receptor in the CT group was 97.2%,and that of the group B was 83.3%,there had statistically significant difference (P<0.05).Incidence of pneumothorax puncture and blood sputum in the CT group were significantly less than the B group (P<0.05).Conclusion CT-guided biopsy in the diagno-sis of lung cancer has better value,it can improve the accuracy of biopsy,reduce the incidence of complications,and it is benefi-cial for the detection of the expression of epidermal growth factor receptor.
