Diabetic retinopathy%Prevention%Combination of TCM and Western medicine%Interven-tion model establishment
目的：通过对糖尿病视网膜病变（DR）的相关指标的筛查，建立中西医结合预防糖尿病视网膜病变的干预模式。方法将2011年6月-2014年6月期间收治的眼科126例（252眼）2型糖尿病患者，随机分成两组，对照组63例采用西医干预模式治疗，治疗组63例在西医方法治疗的基础上，进行中西结合干预模式治疗。结果经过治疗后，对照组的视力提高率为44.44％，治疗组的视力提高率为77.78％，治疗组明显优于对照组，差异有统计学意义（χ2＝14.7273，P ﹤0.05）；且治疗组左右眼的视野灰度显著低于对照组，差异有统计学意义（ t ＝2.9684，P ﹤0.01；t ＝2.1587，P ﹤0.05）；荧光素眼底血管造影结果显示，治疗组微血管瘤数下降程度显著大于对照组，渗出灶面积与对照组比较，差异有统计学意义（P ﹤0.05，P ﹤0.01），两组治疗后出血灶面积均有下降，但治疗组下降更为明显，与治疗前比较，差异有统计学意义（t ＝2.5621，P ﹤0.05）；与对照组比较，差异有统计学意义（t ＝2.5988，P ﹤0.05）；彩色多普勒超声测定结果表明，治疗组的血管阻力指数（RI）的下降程度显著大于对照组，组间比较，差异有统计学意义（P ﹤0.05，P ﹤0.01）。结论采用中西结合干预模式治疗糖尿病视网膜病变效果更为显著，具有重要的临床应用价值。

Objective By screening related indicators on diabetic retinopathy( DR),to establish prevention mode of traditional Chinese medicine and Western medicine in treating diabetic retinopathy. Methods One hundred and twenty six cases(252 eyes)with type 2 diabetes mellitus treated in Department of ophthalmology of our hospital from 2011 June to 2014 June were randomly divided into two groups. Patients in group A(n = 63)were treated with western medicine intervention mode of treatment,group B(n = 63)re-ceived intervention mode of therapy of combining traditional Chinese and western medicine. Curative effects were compared. Results After treatment,visual acuity improvement in group A was 44. 44% ,and that in group B was 77. 78% with a significant difference(χ2 = 14. 7273,P ﹤ 0. 05);vision gray of group B was sig-nificantly lower than that of group A,with a significant difference(t = 2. 968 4,P ﹤ 0. 01;t = 2. 158 7,P ﹤0. 05);fundus fluorescein angiography showed that group B decreased tumor micro - vessels more greatly;oo-zing lesion area of group B was also significantly reduced(P ﹤ 0. 05 or P ﹤ 0. 01);hemorrhage area decreased in both groups,but there was more pronounced decline in group B(t = 2. 562 1,P ﹤ 0. 05);group B hemor-rhage area after treatment was significantly less than that in group A(t = 2. 598 8,P ﹤ 0. 05);color Doppler ultrasound measurement showed that the blood vessels in group B resistance index(RI)of the degree of de-cline was significantly greater than in group A,and there was a significant difference(P ﹤ 0. 05 or P ﹤ 0. 01) between the two groups. Conclusion Compared with only application of western medicine intervention mode of treatment for diabetic retinopathy results,using a combination of Chinese and Western medicine interven-tion mode of therapy effect is more remarkable,which has important value in clinical application.
