目的 探讨原发性开角型青光眼(POAG)视觉通路在磁共振扩散张量成像(DTI)的改变及其特征.方法 临床对照研究.对2011年10月至2013年12月在河北医大第二医院眼科就诊的20例诊断明确的POAG患者与20名年龄匹配的正常志愿者,行OCT、DTI、视野等检查,对比观察POAG与正常对照之间的差异及变化情况.结果 盘周神经纤维层厚度(RNFL) POAG薄于正常组,差异有统计学意义(P<0.05),视神经、视束、视交叉、视放射的各向异性值(FA)均低于对照组差异有统计学意义(P<0.05),相应的表观扩散系数(ADC)则高于对照组差异有统计学意义(P<0.05),外侧膝状体体积小于对照组,差异有统计学意义(P<0.05).结论 POAG在DTI有特征性改变,OCT联合DTI能够发现视觉通路的微观变化,从而为POAG的整体理解提供理论基础,同时为PO-AG早期诊断及时治疗提供有力依据.

Objective To investigate the changes and features of visual pathway with primary open angle glaucoma (POAG) on magnetic resonance diffusion tensor imaging (DTI).Methods The optical coherence tomography (OCT) measurements,DTI results and visual field of POAGs were compared with those of age-matched normal eyes,and analyzed their characteristics.Results The thickness of peripapillary retinal nerve fiber layer with POAG was thinner than that of normal eye with P <0.05.Fractional anisotropy (FA) values of optic nerve,optic tract,optic chiasma and optic radiation of POAGs were lower than those of normal controls (P <0.05).In contrast to FA,apparent dif-fusion coefficients of each section with POAG were higher than those of control ones with P <0.05.The volume of lateral geniculate was significantly smaller than that of normal ones.Conclusions With specific features on DTI,the combination of DTI and OCT make the observation of microstructure changes with POAG possible,which will provide theoretical base and evidence for the early diagnosis and in-time treatment of POAG,and the understanding of the whole picture of PAOG.
