目的 了解肝细胞生长因子(HGF)及其受体(C-MET)在翼状胬肉组织中的异常表达,探讨其对翼状胬肉的作用和影响.方法 临床病例对照研究.对2013年10月至2014年6月在北仑区人民医院眼科,应用免疫荧光组织化学技术对33例翼状胬肉患者切除的翼状胬肉组织进行HGF及C-MET受体的检测,并与15例正常球结膜组织进行对照、分析.结果 免疫荧光组织化学染色显示,在正常球结膜组织、翼状胬肉组织中,HGF及其C-MET受体在鳞状上皮层及上皮下组织中均可见阳性表达,但表达均以鳞状上皮细胞层为主,尤其以上皮层中临近上皮下间质的基底细胞表达最为强烈.15例正常球结膜组织HGF的平均光密度值为0.2644±0.02629,33例翼状胬肉组HGF的平均光密度值为0.3167±0.07221,差异有统计学意义(t=3.658,P<0.05);15例正常球结膜组织C-MET受体的平均光密度值为0.115±0.02086,30例翼状胬肉组C-MET受体的平均光密度值为0.19017±0.039118,差异有统计学意义(t =6.938,P<0.05).结论 翼状胬肉组织中HGF及其C-MET受体表达的异常增加,提示肝细胞生长因子与翼状胬肉的生成发展相关.无论正常球结膜组织或翼状胬肉组织,HGF及其C-MET受体均在鳞状上皮细胞层的基底细胞表达最强烈,故推测:细胞因子在球结膜鳞状上皮层的异常表达及其对上皮下组织的调控是翼状胬肉发生发展中的主要影响因素之一.

Objective To investigate the expression of hepatocyte growth factor (HGF) and its receptor (C-MET) in the tissue of pterygium to discusses their function in the pathogensis of pterygium.Methods Thirty-three pterygium specimens were obtained from 33 pterygium patients during the operations and 15 normal conjunctival specimens were taken from the healthy eyes by the eye bank of the first affiliated hospital,Zhejiang university school of medicine during the keratoplasty.Pathological examinations were performed under the optical microscope.The expression of HGF and C-MET receptor of pterygium and normal conjunctiva were examined using immunofluorescence histochemicaltechnique.The independent sample test was used for statistic analysis.Results The present study showed that the HGF and C-MET receptor were expressed in the epithelial and subepithelial conjunctival tissues of both groups.And the expression of all was mainly in the squamous epithelial cells layer,particularly the basal cells which are adjacent to subepithelial mesenchyme.The average optical density (mean density) of HGF of normal conjunctival tissue was 0.2644 ± 0.02629,while the mean density of pterygium group was 0.3167 ± 0.07221.The expression of HGF between pterygium and normal conjunctival tissues was significantly differently (t =3.658,P <0.05);the mean density of C-MET receptor in normal and pterygium conjunctival tissues was 0.115 ± 0.02086 and 0.19017 ± 0.039118 respectively.The expression of C-MET was significantly difference between the two groups (t =6.938,P <0.05).Conclusions The increased expression of HGF and its C-MET receptor in pterygium indicates that HGF may play an integrated role in the development of pterygium.Further more,there gulating effect of cytokines to subepithelial tissues,which is abnormally expressed from the squamous epithelial cell layer,may be one of the main influencing factors of pterygium development.
