目的 探索硫酸镁联合硝苯地平用于妊娠期高血压疾病( PIH)治疗时对患者血小板活性物质、抗氧化能力及血液流变学的影响. 方法 收集2013年7月至2014年7月经乐清市妇幼保健院确诊并接受治疗的130例PIH患者进行研究,按照就诊顺序被随机分作两组,各65例. 对照组为单纯采用硫酸镁治疗,观察组则采用硫酸镁联合硝苯地平进行治疗,对患者治疗前后的血小板活性物质、抗氧化能力及血液流变学各项指标进行检测和比较分析. 结果 经治疗后观察组孕妇的血小板聚集率(PAgT)、血小板α颗粒膜糖蛋白(CD62P)、血小板溶酶体膜糖蛋白(CD63)和血小板表面膜糖蛋白Ⅱb/Ⅲa(GPⅡb/Ⅲa)与对照组比较均显著降低,差异均有统计学意义(t值分别为7.885、8.560、8.944、9.139,均P<0.05);经治疗后观察组孕妇的总抗氧化能力( TAC)、超氧化物歧化酶( SOD)水平与对照组比较均显著升高,丙二醛( MDA)及脂质过氧化物( LOP)水平均显著降低,差异均有统计学意义(t值分别为6.877、7.467、6.899、7.310,均P<0.05);治疗后观察组的血流变学全血黏度高切、全血黏度低切、血浆黏度、红细胞压积指标与对照组比较均显著降低,差异均有统计学意义(t值分别为6.833、6.640、7.202、6.630,均P<0.05);观察组产后出血、产后子痫、胎盘早剥、早产、新生儿窒息及围产儿死亡发生率较对照组均显著降低( t值分别为8.434、8.822、9.732、8.709、7.322、6.563,均P<0.05). 结论 硫酸镁联合硝苯地平用于PIH治疗效果显著,能够有效改善患者的血小板活性物质、抗氧化能力及血液流变学性,值得临床借鉴和应用.

Objective To explore the influence of Magnesium Sulfate combined with Nifedipine on platelet active substances, antioxidant capacity and blood rheology of patients with pregnancy induced hypertension ( PIH) .Methods From July 2013 to July 2014 130 cases of PIH diagnosed and treated in Yueqing Maternal and Child Health Hospital were collected, and they were divided into two groups with 65 cases in each according to the order of treatment.The control group was given Magnesium Sulfate treatment only, while the observation group was given Magnesium Sulfate combined with Nifedipine treatment. The platelet active substances, antioxidant ability and hemorheologic indexes before and after treatment were observed and compared.Results After treatment, PAgT, CD62P, CD63 and GPⅡb/Ⅲa of the observation group decreased significantly compared with those of the control group, and the differences were significant ( t value was 7.885, 8.560, 8.944 and 9.139, respectively, all P<0.05).TAC and SOD levels of the observation group after treatment were significantly increased but MDA and LOP levels decreased significantly compared with the control group, and the differences were significant (t value was 6.877,7.467,6.899 and 7.310, respectively, all P <0.05).After treatment, the blood rheology indexes including whole blood viscosity (in low shear rate and in high shear rate), plasma viscosity and hct were decreased in the observation group, and the differences were significant (t value was 6.833, 6.640, 7.202 and 6.630, respectively, all P<0.05).The incidences of postpartum hemorrhage, postpartum eclampsia, placental abruption, preterm delivery, asphyxia and perinatal mortality of the observation group were significantly lower than the control group (t value was 8.434, 8.822, 9.732, 8.709, 7.322 and 6.563, respectively, all P<0.05).Conclusion Magnesium Sulfate combined with Nifedipine for PIH has significant effect, and it can effectively improve platelet active substances, antioxidant capacity and blood rheology.It is worthy of clinical reference and application.
