目的 了解西安市某社区女性行为因素,揭示其行为影响因素与常见慢性病的关系. 方法 采用横断调查设计,随机整群抽样,对西安市某社区1 877户居民15岁以上常住人口进行问卷调查. 结果 该社区女性不吃早餐、运动不足、喜食重盐和烟熏食物人群比率分别为5.42%、30.14%、13.64%和5.04%. 常见慢性疾病为高血压、糖尿病、冠心病和脑卒中,患病率分别为15.93%、4.72%、1.75%、2.22%,其慢性病患病率随着年龄的增加而增高,30岁后的慢性病患病率增加更为明显,且糖尿病与高血压的患病呈明显的正相关性(r=0.998,P=0.002). 高龄、文化程度越高(静坐工作时间长)、BMI>24kg/m2 是该社区女性居民高血压的主要危险因素[OR(95%CI)分别为1.095(1.08,1.111)、2.464(1.214, 5.001)、2.287(1.535, 3.407),均P<0.05]. 高龄是该社区居民糖尿病的主要危险因素[OR(95%CI)为1.072 (1.049, 1.096),P<0.05]. 高龄、运动不足是该社区居民患脑卒中的主要危险因素[OR(95%CI)分别为1.097(1.064, 1.132)、9.288(2.326,33.427),均P<0.05],而低盐饮食是脑卒中的保护因素[OR(95%CI)为0.338(0.167, 0.687),P<0.05]. 高龄、BMI>24、乳制品摄入较少、静态行为时间>6h、运动不足是该社区女性居民常见慢性病发病的主要危险因素[OR(95%CI)分别为1.103(1.077, 1.130)、1.852(1.184, 2.896)、1.137 (1.098, 1.186)、1.424(1.187, 1.961)、1.763(1.176, 2.693),均P<0.05],而每日蔬菜水果量>400g则是防控该社区居民常见慢性病发病的主要保护因素( OR=0.460,P<0.05). 结论 高龄、BMI>24、静态行为时间长、运动不足是该社区女性居民慢性病患病的主要危险因素,每日摄入400 g以上蔬菜水果、低盐饮食是其有效的保护因素.

Objective To study the behavior factors of the females in a community in Xi' an City and discover the relationship between these factors and chronic diseases in females.Methods Cross-section survey design and random cluster sampling were used to conduct the questionnaire survey among residents aged 15 and above in 1 877 families in the community.Results The proportions of the females with breakfast skipping, exercise refusing, favoring salty food and smoked foods were 5.42%, 30.14%, 13.64% and 5.04%, respectively. The common chronic diseases of the female residents in the community were hypertension, diabetes, coronary heart disease and stroke, and their morbidity rates were 15.93%, 4.72%, 1.75% and 2.22%, respectively.With the age increasing, the prevalence of chronic diseases increased, and the increasing after 30 years old was significant.An obvious correlation existed between the prevalence of diabetes and hypertension ( r=0.998, P=0.002) .The major risk factors of hypertension of the female residents in the community were advanced age, higher education (longer sedentary work time) and BMI>24 [OR (95%CI) was 1.095(1.08, 1.111), 2.464(1.214, 5.001) and 2.287(1.535, 3.407), respectively,P<0.05].Advanced age was the main risk factor of diabetes [OR (95%CI) was 1.072 (1.049, 1.096), P<0.05].Advanced age and lack of exercise were the main risk factors of stroke [OR (95%CI) was 1.097(1.064, 1.132) and 9.288(2.326, 33.427), respectively,P<0.05], while light salinity food was the protective factor of stroke [OR (95%CI) was 0.338(0.167, 0.687),P<0.05].Advanced age, BMI>24, less dairy consumption, time of static behavior>6 hours and lack exercise were the main risk factors of chronic diseases of the female residents in the community [OR (95%CI) was 1.103(1.077, 1.130), 1.852(1.184, 2.896), 1.137(1.098, 1.186), 1.424(1.187, 1.961) and 1.763(1.176, 2.693), respectively, all P<0.05].Daily vegetable intake>400g was the major protective factor of chronic disease (OR=0.460,P<0.05).Conclusion Advanced age, BMI>24, long time for static behavior and lack of exercise are the main risk factors of chronic diseases in female residents of the community, but vegetable intake above 400g per day and light salinity food are the effective protective factors.
