目的 分析全髋关节置换治疗强直性脊柱炎髋关节强直的临床效果.方法 选择我院2010年1月~2012年6月收治的行人工全髋关节置换术的强直性脊柱炎患者38例(43髋),对其随访3年,比较手术前后Harris评分,根据Gruen股骨分区法和DeLee-Charnley髋臼分区法对股骨假体和髋臼假体作影像学分析.结果 术后患者Harris评分为(86.87±2.86)分高于术前的(28.23±2.18)分,优27髋,良12髋,可3髋,差1髋,优良率为90.70%.术前髋关节总活动度由(36.53±3.61)°提高至术后的(187.73±6.28)°;X线检查未见假体松动、脱位以及折断的现象;异位骨化发生率为11.63%(5髋).结论 全髋关节置换术治疗强直性脊柱炎髋关节强直可恢复患者的关节功能.

Objective To analysis the clinical effect of total hip replacement in the treatment of ankylosing spondylitis.Methods Selected 38 patients with ankylosing spondylitis ( 43 hips ) treated with pedestrian total hip arthroplasty from January 2010 to June 2012 in our hospital. Follow-up for 3 years and compared harris score before and after surgery. Image analysis of femoral and acetabular according to DeLee-Charnley based on the Gruen method and the femoral partition method.Results The Harris score of patients was ( 86.87±2.86 ), which was higher than that of the patients ( 28.23±2.18 ), there were 27 hips was excellent, 12 hips was good, 3 hips was ok,1 hips was bad, the excellent and good rate was 90.70%. Preoperative total activity of hip joint by( 36.53±3.61 )°improve after surgery ( 187.73±6.28 )°, X-ray examination showed that there were no prosthesis loosening, dislocation and broken phenomenon. Heterotopic ossification occurred rate was 11.63% ( 5 hips ).Conclusion Total hip arthroplasty in treatment of patients with ankylosing spondylitis hip joint stiffness can be recovery of the joint function of patients.
