目的：研究血府逐瘀胶囊联合双水平无创正压通气治疗老年慢性肺心病并发Ⅱ型呼吸衰竭对患者预后及血小板聚集的影响。方法选择2013年1月—2014年9月间我院收治的122例慢性肺心病并发Ⅱ型呼吸衰竭患者，随机分为为观察组（61例）和对照组（61例），2组患者均行常规治疗并加以双水平无创正压通气治疗；观察组在此基础上加服血府逐瘀胶囊。对比2组治疗效果及治疗前后的血小板聚集情况。结果观察治疗总有效率高达96.72％，显著高于对照组中的85.25％（ P＜0.05）；观察组患者的平均住院时间（10.8±5.1） d显著短于对照组（13.2±5.9） d ，2组比较差异有统计学意义（ P＜0.05）。2组患者的病死率相比虽然没有显著差异，但是观察组中的病死率仅为1.64％，低于对照组。2组患者在治疗前后，其血小板的聚集率均有明显的降低，与治疗前相比差异具有统计学意义（ P＜0.05）。观察组治疗后血小板聚集率显著低于对照组，2组相比差异具有统计学意义（ P＜0.05）。结论血府逐瘀胶囊联合双水平无创正压通气治疗老年慢性肺心病并发Ⅱ型呼吸衰竭，能够有效的改善患者预后，降低血小板聚集。
목적：연구혈부축어효낭연합쌍수평무창정압통기치료노년만성폐심병병발Ⅱ형호흡쇠갈대환자예후급혈소판취집적영향。방법선택2013년1월—2014년9월간아원수치적122례만성폐심병병발Ⅱ형호흡쇠갈환자，수궤분위위관찰조（61례）화대조조（61례），2조환자균행상규치료병가이쌍수평무창정압통기치료；관찰조재차기출상가복혈부축어효낭。대비2조치료효과급치료전후적혈소판취집정황。결과관찰치료총유효솔고체96.72％，현저고우대조조중적85.25％（ P＜0.05）；관찰조환자적평균주원시간（10.8±5.1） d현저단우대조조（13.2±5.9） d ，2조비교차이유통계학의의（ P＜0.05）。2조환자적병사솔상비수연몰유현저차이，단시관찰조중적병사솔부위1.64％，저우대조조。2조환자재치료전후，기혈소판적취집솔균유명현적강저，여치료전상비차이구유통계학의의（ P＜0.05）。관찰조치료후혈소판취집솔현저저우대조조，2조상비차이구유통계학의의（ P＜0.05）。결론혈부축어효낭연합쌍수평무창정압통기치료노년만성폐심병병발Ⅱ형호흡쇠갈，능구유효적개선환자예후，강저혈소판취집。
Objective To investigate the effect of Xuefuzhuyu capsules in combined with bi-level positive airway pressure ventilation on the prognosis and platelet aggregation in elder patients with chronic pulmonary heart disease complicated by type II respiratory failure .Methods A total of 122 patients with chronic pulmonary heart disease complicated by type II respiratory failure who were admitted in our hospital from January ,2013 to September ,2014 were included in the study and randomized into the observation group ( n=61 ) and the control group ( n=61 ) .The patients in the two groups were given the routine treatment in combined with bi-level positive airway pressure ventilation .On the above basis ,the patients in the observation group were additionally administered with Xuefuzhuyu capsules .The therapeutic effect and the platelet aggrega-tion before and after treatment in the two groups were compared .Results The total effective rate in the observation group (96 .72% ) was significantly higher than that in the control group (85 .25% ) ( P<0 .05 ) .The average hospitalization time in the observation group (10 .8 ± 5 .1 ) d was significantly shorter than that in the control group (13 .2 ± 5 .9 ) d ( P<0 .05 ) .The comparison of the mortality between the two groups was not statistically significant ,but the mortality in the observation group was only 1 .64% ,lower than that in the control group .The platelet aggregation rate after treatment in the two groups was significantly reduced when compared with that before treatment ( P<0 .05 ) .The platelet aggregation rate af-ter treatment in the observation group was significantly lower than that in the control group ( P<0 .05 ) .Conclusions Xue-fuzhuyu capsules in combined with bi-level positive airway pressure ventilation in the treatment of elder patients with chronic pulmonary heart disease complicated by type II respiratory failure can effectively improve the prognosis ,and reduce the platelet aggregation with a favorable effect .
