目的:探讨红细胞分布宽度( RDW)对重叠综合征的预测价值。方法连续收集2013年6月—2015年1月在暨南大学第二临床医学院住院治疗的20例轻度COPD患者( A组)、20例中重度COPD患者( B组)、20例轻度重叠综合征患者( C组)及20例中重度重叠综合征患者( D组),并根据RDW参考范围将所有患者分为RDW正常组59例和RDW增高组21例。比较A、 B、 C、 D 4组患者一般资料、动脉血气分析指标〔pH值、动脉血氧分压( PaO2)、动脉血二氧化碳分压( PaCO2)〕、血浆超敏C反应蛋白( hs-CRP)水平及RDW;比较RDW正常组与RDW增高组患者一般资料、动脉血气分析指标及血浆hs-CRP水平;并分析RDW与PaO2、血浆hs-CRP水平的相关性。结果4组患者性别、年龄及pH值比较,差异无统计学意义(P>0.05); A组和B组患者PaO2高于D组,血浆hs-CRP水平和RDW低于D组, C组患者PaCO2低于D组, A组和B组患者RDW低于C组(P<0.05)。 RDW正常组与RDW增高组患者性别、年龄、 pH值及PaCO2比较,差异无统计学意义( P>0.05); RDW增高组患者血浆hs-CRP水平高于RDW正常组、 PaO2低于RDW正常组(P<0.05)。 Pearson相关分析结果显示, RDW与PaO2呈负相关(r=-0.265, P=0.018),与血浆hs-CRP水平呈正相关(r=0.258, P=0.021)。结论 RDW可用于重叠综合征患者炎症及气道病变严重程度的初步评估,具有一定的预测价值。

목적:탐토홍세포분포관도( RDW)대중첩종합정적예측개치。방법련속수집2013년6월—2015년1월재기남대학제이림상의학원주원치료적20례경도COPD환자( A조)、20례중중도COPD환자( B조)、20례경도중첩종합정환자( C조)급20례중중도중첩종합정환자( D조),병근거RDW삼고범위장소유환자분위RDW정상조59례화RDW증고조21례。비교A、 B、 C、 D 4조환자일반자료、동맥혈기분석지표〔pH치、동맥혈양분압( PaO2)、동맥혈이양화탄분압( PaCO2)〕、혈장초민C반응단백( hs-CRP)수평급RDW;비교RDW정상조여RDW증고조환자일반자료、동맥혈기분석지표급혈장hs-CRP수평;병분석RDW여PaO2、혈장hs-CRP수평적상관성。결과4조환자성별、년령급pH치비교,차이무통계학의의(P>0.05); A조화B조환자PaO2고우D조,혈장hs-CRP수평화RDW저우D조, C조환자PaCO2저우D조, A조화B조환자RDW저우C조(P<0.05)。 RDW정상조여RDW증고조환자성별、년령、 pH치급PaCO2비교,차이무통계학의의( P>0.05); RDW증고조환자혈장hs-CRP수평고우RDW정상조、 PaO2저우RDW정상조(P<0.05)。 Pearson상관분석결과현시, RDW여PaO2정부상관(r=-0.265, P=0.018),여혈장hs-CRP수평정정상관(r=0.258, P=0.021)。결론 RDW가용우중첩종합정환자염증급기도병변엄중정도적초보평고,구유일정적예측개치。
Objective To investigate the predictive value of red cell distribution width ( RDW) in patients with COPD-OSAHS overlap syndrome.Methods From June 2013 to January 2015 in the Second Clinical College of Ji′nan University, 20 inpatients with mild COPD were selected as A group, 20 inpatients with moderate and severe COPD were selected as B group, 20 inpatients with mild COPD-OSAHS overlap syndrome were selected as C group, 20 inpatients with moderate and severe COPD-OSAHS were selected as D group, and they were also divided into E group (with normal RDW, n=59) and F group ( with increased RDW, n =21 ) according to RDW reference range.General information, arterial blood -gas analysis index ( including pH, PaO2 , PaCO2 ) , plasma hs-CRP level and RDW were compared among the four groups ( A, B, C and D); general information, arterial blood-gas analysis index ( including pH, PaO2 , PaCO2 ) and plasma hs-CRP level were compared between the two groups ( E and F); correlations between RDW and PaO2 , plasma hs-CRP level was respectively analyzed.Results No statistically significant differences of gender, age or pH was found among the four groups ( P>0.05);PaO2 of A group and B group was statistically significantly higher than that of D group, respectively, plasma hs-CRP level of A group and B group was statistically significantly lower than that of D group, respectively, PaCO2 of C group was statistically significantly lower than that of D group, RDW of A group and B group was statistically significantly higher than that of C group, respectively (P<0.05).No statistically significant differences of gender, age, pH or PaCO2 was found between the two groups ( P>0.05); plasma hs-CRP level of F group was statistically significantly higher than that of E group, while PaO2 of F group was statistically significantly lower than that of E group ( P <0.05 ) . Pearson correlation analysis showed that, RDW was negatively correlated with PaO2 (r=-0.265, P=0.018), was positively correlated with plasma hs-CRP level (r=0.258, P=0.021) .Conclusion RDW can be used in the preliminary evaluation of inflammation and airway disease severity in patients with COPD-OSAHS overlap syndrome, has certain predictive value.
