目的：探讨结肠癌患者血清脂肪因子水平变化的临床意义。方法选择结肠癌患者96例（结肠癌组），其中 Dukes 病理临床分期 A 期26例，B 期20例，C 期28例，D 期22例。选择同期健康体检的30例健康成人作为对照组。采用免疫酶联吸附法（ELISA）法检测血清脂联素、内脂素、抵抗素、瘦素水平。分析结肠癌患者血清脂肪因子的变化以及与 Dukes 病理分期的关系、不同浓度脂肪因子的结肠癌患者3年生存率的差别。结果与对照组比较，结肠癌患者血清抵抗素、内脂素和瘦素水平升高，脂联素水平降低（均 P ＜0．05）。与 Dukes 分期 A—B 期患者比较，C—D 期患者血清抵抗素、内脂素和瘦素水平明显升高，脂联素水平明显下降（均 P ＜0．05）。低质量浓度和高质量浓度内脂素结肠癌患者3年累积生存率分别为80．52％和65．30％（P ＝0．044）；低质量浓度和高质量浓度瘦素结肠癌患者3年累积生存率分别为82．60％和64．30％（P ＝0．049）；低质量浓度和高质量浓度抵抗素结肠癌患者3年累积生存率分别为80．43％和60．00％（P ＝0．0436）；低质量浓度和高质量浓度脂联素结肠癌患者3年累积生存率分别为60．20％和80．50％（P ＝0．0266）。结论血清脂肪因子的检测对结肠癌患者具有病情判断和预后评估的价值。

ABSTRACT:Objective To explore the clinical significance of changes in serum adipocytokine levels in patients with colon cancer.Methods Serum levels of adiponectin,visfatin,resistin and leptin were detected by ELISA in 96 patients with colon cancer(Dukes’stage A in 26,Dukes’ stage B in 20,Dukes’stage C in 28 and Dukes’stage D in 22)and 30 healthy controls.The rela-tionship between serum adipocytokine levels and Dukes’stages was analyzed.In addition,the 3-year survival rate was compared among patients with different adipocytokine concentrations.Re-sults Compared with healthy controls,serum levels of resistin,visfatin and leptin increased and serum levels of adiponectin decreased in patients with colon cancer(P <0.05).Compared with pa-tients with Dukes’stage A or B colon cancer,serum levels of resistin,visfatin and leptin increased and serum levels of adiponectin decreased in patients with Dukes’stage C or D colon cancer(P <0.05).The 3-year survival rates were,respectively,80.52% and 65.30% in patients with low and high concentrations of visfatin(P =0.044),82.60% and 64.30% in patients with low and high concentrations of leptin(P =0.049),80.43 and 60.00% in patients of low and high concentrations of resistin(P =0.043 6),and 60.20% and 80.50% in patients with low and high concentrations of adiponectin(P =0.026 6).Conclusion The measurement of serum adipocytokines is valuable for condition judgment and prognosis evaluation in patients with colon cancer.
