目的：了解常规血液透析（HD）、高通量血液透析（HFD）、联机血液透析滤过（HDF）对终末期肾病维持性血液透析（MHD）患者微炎症状态的影响。方法选择血透室透析龄超过1年的 MHD 患者60例（患者血液 CRP、IL-6水平均高于正常），将其按随机数字表法分为 HD 组、HFD 组、HDF 组，每组20例。分别检测3组患者首次透析前（T前）、首次透析后（T后）及透析2个月后（T2个月）的 CRP、IL-6水平。结果与 T前比较，HDF 组 T2个月 CRP 明显下降（P ＜0．05），HFD 组 T2个月、HDF 组 T后及 T2个月 IL-6均明显下降（P ＜0．05），HDF 组 T2个月 CRP、IL-6下降百分比高于 HFD 组（P ＜0．01）。结论MHD 患者存在微炎症状态，HDF 及 HFD 较 HD 可减轻患者体内微炎症状态，且 HDF 的效果更好。
목적：료해상규혈액투석（HD）、고통량혈액투석（HFD）、련궤혈액투석려과（HDF）대종말기신병유지성혈액투석（MHD）환자미염증상태적영향。방법선택혈투실투석령초과1년적 MHD 환자60례（환자혈액 CRP、IL-6수평균고우정상），장기안수궤수자표법분위 HD 조、HFD 조、HDF 조，매조20례。분별검측3조환자수차투석전（T전）、수차투석후（T후）급투석2개월후（T2개월）적 CRP、IL-6수평。결과여 T전비교，HDF 조 T2개월 CRP 명현하강（P ＜0．05），HFD 조 T2개월、HDF 조 T후급 T2개월 IL-6균명현하강（P ＜0．05），HDF 조 T2개월 CRP、IL-6하강백분비고우 HFD 조（P ＜0．01）。결론MHD 환자존재미염증상태，HDF 급 HFD 교 HD 가감경환자체내미염증상태，차 HDF 적효과경호。
Objective To understand the effects of conventional hemodialysis(HD),high-flux hemodialysis(HFD)and online hemodiafiltration(HDF)on microinflammatory state in mainte-nance hemodialysis(MHD)patients with end-stage renal disease(ESRD).Methods Sixty MHD patients with blood C-reactive protein(CRP)and interleukin-6(IL-6)levels higher than normal who were hemodialyzed over 1 year were randomly divided into three groups:HD group,HFD group and HDF group,with 20 patients in each group.The levels of CRP and IL-6 were detected before the first hemodialysis,after the first hemodialysis and after hemodialysis for 2 months.Re-sults The CRP levels decreased after hemodialysis for 2 months in HDF group(P <0.05).The IL-6 levels decreased after hemodialysis for 2 months in HFD group,as well as after the first he-modialysis and after hemodialysis for 2 months in HDF group(P <0.05).Compared with HFD group,the decreased percentages of CRP and IL-6 increased in HDF group(P <0.01).Conclusion Microinflammatory state exists in MHD patients.Compared with HD,HDF and HFD,especially HDF,can reduce the microinflammation in MHD patients.
