目的：探讨常规 MRI 联合扩散加权成像（DWI）在软组织未分化多形性肉瘤诊断中的价值。方法回顾性分析16例经手术和病理确诊的软组织未分化多形性肉瘤患者的 MRI 影像及病理资料。结果16例软组织未分化多形性肉瘤患者中发生于大腿11例，小腿2例，臀部2例，上臂1例，均为单发病灶。16例肿瘤形态多样，其中14例可见不规则分叶。16例肿瘤中13例 T1 WI 呈稍低信号或等信号，3例呈混杂稍高信号；T2 WI 及脂肪抑制序列均呈混杂高信号，其中8例可见分隔征，8例可见假包膜，9例可见瘤周水肿；增强扫描肿瘤实质部分呈明显不均匀强化；DWI 均呈高信号。结论常规 MRI 联合 DWI 对软组织未分化多形性肉瘤具有一定的诊断价值。
목적：탐토상규 MRI 연합확산가권성상（DWI）재연조직미분화다형성육류진단중적개치。방법회고성분석16례경수술화병리학진적연조직미분화다형성육류환자적 MRI 영상급병리자료。결과16례연조직미분화다형성육류환자중발생우대퇴11례，소퇴2례，둔부2례，상비1례，균위단발병조。16례종류형태다양，기중14례가견불규칙분협。16례종류중13례 T1 WI 정초저신호혹등신호，3례정혼잡초고신호；T2 WI 급지방억제서렬균정혼잡고신호，기중8례가견분격정，8례가견가포막，9례가견류주수종；증강소묘종류실질부분정명현불균균강화；DWI 균정고신호。결론상규 MRI 연합 DWI 대연조직미분화다형성육류구유일정적진단개치。
Objective To investigate the value of conventiongal MRI in combination with diffusion weighted imaging in the diagnosis of undifferentiated pleomorphic sarcoma of soft tissue.Methods The MRI imaging data of 16 patients confirmed by operation and pathology in the radiology department of Henan Cancer Hospital were retrospectively analyzed.Results In the 16 cases of undifferentiated pleomorphic sarcoma,11 cases were located in thigh,2 cases in calf,2 cases in buttock and 1 case in upper arm,all of them were single lesion.The MRI appearance varied in different cases,and 14 of them were irregular and lobu-lated mass.13 cases were shown slight hypo -intense or iso -intense signal on T1 WI,while 3 were heterogeneous high signal. All of the 16 cases were shown heterogeneous high signal on T2 WI and fat suppression sequence.8 cases had separated sign,8 cases had pseudomembrane,and 9 cases had peritumoral edema.The tumors were enhanced obviously and unequally.All of them were high signal on DWI.Conclusion Conventional MRI in combination with DWI can be very valuable for the diagnosis of undifferentiated pleomorphic sarcoma of soft tissue.
