目的:通过对口腔本科生手工龈下刮治术的培训效果进行评价和分析,为提高牙周临床培训的效果提供参考。方法:对65名口腔医学系大四本科生进行8个学时的手工龈下刮治术的培训,培训过程中学生用Gracey刮治器在仿头模结合带有人工牙石的仿真牙列模型上进行操作,培训结束后指导教师对学生操作的要点、体位和刮治的效果进行评价和分析。结果:无论是对龈下刮治要点的掌握还是刮治的最终效果匀是前牙区好于后牙区,学生对器械型号、工作端及工作面的选择、器械的握持方式、用力方向和幅度掌握较好,对支点、角度、用力方式和体位掌握不佳。前牙区刮治后残留牙石的位点主要集中在袋底,后牙区残留牙石的位点比较分散,但多集中于根分叉,根面轴角和邻面接触点下方。结论:在进行手工龈下刮治术的培训中,指导教师应加强对学生支点、角度、用力方式和体位的培训,同时指导学生对残留牙石集中的部位进行重点检查,在条件允许的情况下学校应提供更多的时间和空间方便学生练习。

목적:통과대구강본과생수공간하괄치술적배훈효과진행평개화분석,위제고아주림상배훈적효과제공삼고。방법:대65명구강의학계대사본과생진행8개학시적수공간하괄치술적배훈,배훈과정중학생용Gracey괄치기재방두모결합대유인공아석적방진아렬모형상진행조작,배훈결속후지도교사대학생조작적요점、체위화괄치적효과진행평개화분석。결과:무론시대간하괄치요점적장악환시괄치적최종효과균시전아구호우후아구,학생대기계형호、공작단급공작면적선택、기계적악지방식、용력방향화폭도장악교호,대지점、각도、용력방식화체위장악불가。전아구괄치후잔류아석적위점주요집중재대저,후아구잔류아석적위점비교분산,단다집중우근분차,근면축각화린면접촉점하방。결론:재진행수공간하괄치술적배훈중,지도교사응가강대학생지점、각도、용력방식화체위적배훈,동시지도학생대잔류아석집중적부위진행중점검사,재조건윤허적정황하학교응제공경다적시간화공간방편학생연습。
Objective:Evaluate the results of subgingival scaling with curettes by undergradulate students and analyze the associated factors in order to improve the result of periodontal clinical training. Method:sixty-five dental students from stomatology school China Medical University who finish their courses and enter their pre-clinical training were recruited , teacher showed them how to perform subgingival scaling with curettes and told them the manual skills ,students practice subgingival scaling on generic head model with artificial tooth using Gracey curettage ,after that teacher evaluate how they grasped the key points manual skills and record the results of subgingival scaling. Result:Students mastered more manual skills when they performed anterior teeth than molar. It is easier for student to master skills about how to choose the num-ber,tips and correct cutting edge of the curette,modified pen grasp,forcing direction and extent,but difficult to master the stills of fulcrum,working angulation,wrist-arm motion and operation position. After subgingival scaling,the result of instru-mentation is better for anterior teeth than molar. Remaining subgingival deposits were encountered frequently at the base of pocket in anterior teeth and furcation,root line angles and midproximal region apical to the contact in molar. Conclusion:During periodontal pre-clinical training,teacher should emphasize the skills of fulcrum,working angulation,wrist-arm mo-tion and operation position and indicate the students to detect the position where easy to remain subgingival deposits. Dental school should provide more opportunity for students to practice subgingival scaling.
