目的:探讨 MRI-DWI 在早期大鼠肝泡球蚴病模型中的应用价值。方法对接种9 w 并经超声筛查确认接种成功的40只 SD 大鼠进行 MRI-DWI 检查,记录病灶在 MRI-T1、T2以及 DWI 图像上的特点,并与病理结果进行对照。结果早期大鼠肝泡状棘球蚴在 T1WI 图像上表现为单个或多个类圆形低信号,T2及 DWI 图像上均表现为单个或多个类圆形高信号,DWI 成像中病灶周边可见稍低于病灶内部的环形区,类似“月晕”征。病灶边缘区的 ADC 值较病灶内部及邻近正常肝组织高(P <0.05)。HE 染色镜下见单个或多个囊泡样红染结构,正常肝组织交界区富含微血管,伴炎性细胞浸润。结论早期大鼠肝泡球蚴病灶周边“月晕区”可作为 DWI 诊断该病较特异的依据,该区域的 ADC 值高于病灶内部及正常肝实质,经病理证实该区域为富含微血管的区域。

목적:탐토 MRI-DWI 재조기대서간포구유병모형중적응용개치。방법대접충9 w 병경초성사사학인접충성공적40지 SD 대서진행 MRI-DWI 검사,기록병조재 MRI-T1、T2이급 DWI 도상상적특점,병여병리결과진행대조。결과조기대서간포상극구유재 T1WI 도상상표현위단개혹다개류원형저신호,T2급 DWI 도상상균표현위단개혹다개류원형고신호,DWI 성상중병조주변가견초저우병조내부적배형구,유사“월훈”정。병조변연구적 ADC 치교병조내부급린근정상간조직고(P <0.05)。HE 염색경하견단개혹다개낭포양홍염결구,정상간조직교계구부함미혈관,반염성세포침윤。결론조기대서간포구유병조주변“월훈구”가작위 DWI 진단해병교특이적의거,해구역적 ADC 치고우병조내부급정상간실질,경병리증실해구역위부함미혈관적구역。
Objective This study was to demonstrate characteristic imaging and accurate evaluation in early stage of hepatic alveolar echinococcosis (HAE)by Diffusion-weighted MRI (DWI).Methods The early stage of experimentally induced secondary HAE in 40 rats was studied.Forty HAE lesions in 40 rats,con-firmed by Ultrasonography,were examined by MRI-T1,T2 and DWI.Results The early stage of experi-mentally induced secondary HAE presented low signal on MRI-T1,single or multiple round high signal on MRI-T2.The “halo”sign surrounding lesion was shown on DWI,and the ADC value of “halo”belt was higher than the centre of lesion and the normal hepatic tissue (P <0.05).Hematoxylin-eosin (HE)stai-ning showed that the small cyst was surrounded by the characteristically proliferated small blood vessels and infiltrated inflammatory cells.Conclusion The “halo zone”surrounding the lesions is the typical feature in DWI for the early stage of experimentally induced secondary HAE in rats,and the higher ADC value re-presented the inflammatory response belt with microvascular.
