目的:探讨细胞因子IL-6、IFN-γ、TNF-α在慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)合并肺结核(pulmonary tuberculosis,TB)疾病发生发展过程中的作用和意义。方法:采用双抗体夹心ELISA方法测定2011年3月-2013年12月本院呼吸内科收治入院治疗的COPD合并TB患者(n=152)、TB患者(n=150)、COPD患者(n=157)和正常健康人(n=50)的血清细胞因子IL-6、IFN-γ、TNF-α水平变化情况。应用SPSS 13.0统计学软件对实验数据进行分析。结果:与健康对照组相比,COPD合并TB组以及TB组患者的IL-6、TNF-α、IFN-γ水平均显著升高,差异均有统计学意义(P<0.05);与COPD组及TB组相比,COPD合并TB组患者IL-6、TNF-α、IFN-γ水平均显著升高,差异均有统计学意义(P<0.05);而COPD组与TB组患者的IL-6、TNF-α、IFN-γ水平比较差异均无统计学意义(P>0.05)。结论:COPD合并TB患者血清细胞因子IL-6、IFN-γ、TNF-α水平存在不同程度的失衡,提示其在COPD合并肺结核的发生发展中起着重要作用,为免疫干预治疗提供一定的理论依据。

목적:탐토세포인자IL-6、IFN-γ、TNF-α재만성조새성폐질병(chronic obstructive pulmonary disease,COPD)합병폐결핵(pulmonary tuberculosis,TB)질병발생발전과정중적작용화의의。방법:채용쌍항체협심ELISA방법측정2011년3월-2013년12월본원호흡내과수치입원치료적COPD합병TB환자(n=152)、TB환자(n=150)、COPD환자(n=157)화정상건강인(n=50)적혈청세포인자IL-6、IFN-γ、TNF-α수평변화정황。응용SPSS 13.0통계학연건대실험수거진행분석。결과:여건강대조조상비,COPD합병TB조이급TB조환자적IL-6、TNF-α、IFN-γ수평균현저승고,차이균유통계학의의(P<0.05);여COPD조급TB조상비,COPD합병TB조환자IL-6、TNF-α、IFN-γ수평균현저승고,차이균유통계학의의(P<0.05);이COPD조여TB조환자적IL-6、TNF-α、IFN-γ수평비교차이균무통계학의의(P>0.05)。결론:COPD합병TB환자혈청세포인자IL-6、IFN-γ、TNF-α수평존재불동정도적실형,제시기재COPD합병폐결핵적발생발전중기착중요작용,위면역간예치료제공일정적이론의거。
Objective:To investigate the change and significance of cytokines IL-6,IFN-γ,TNF-α in the patients with chronic obstructive pulmonary disease complicated with pulmonary tuberculosis.Method:The serum levels of cytokines IL-6,IFN-γ,TNF-α in the patients with COPD complicated with TB were measured by double antibody sandwich enzyme-linked immunosorbent assay(ELISA),whose patients were including 152 patients with COPD complicated by pulmonary tuberculosis(COPD and TB group),150 patients with pulmonary tuberculosis (TB group), as well as 157 patients with COPD(COPD group) and 50 healthy volunteers (healthy control group) in the department of respiratory medicine of our hospital from March 2011 to December 2013 were selected.The data were analyzed by SPSS 13.0 statistic software.Result:Compared to the healthy control group,the serum levels of cytokines IL-6,IFN-γ, TNF-α in the patients with COPD complicated with TB and TB group were increased,the differences were statistically significant(P<0.05).Compared to the COPD and TB group,the serum levels of cytokines IL-6,IFN-γ,TNF-αin the patients with COPD complicated with TB group were increased,the differences were statistically significant (P<0.05),however,there were no statistically significant differences in the serum levels of cytokines IL-6,IFN-γ, TNF-α between the patients with COPD and TB group(P>0.05).Conclusion:There are different extents of imbalance of some serum cytokines IL-6,IFN-γ,TNF-α in the patients with COPD complicated by TB.It suggests that there are important roles of these cytokines in immune pathogenesis of COPD complicated with TB and will provide theoretical evidence to immune intervention.
