目的:探讨逆行髓内钉与钢板螺钉治疗儿童股骨干骨折的效果。方法:回顾性分析51例儿童股骨干骨折手术治疗方法及结果。结果:逆行髓内钉固定33例,钢板螺钉固定18例,随访5个月~4年5个月(平均2年6个月),均在4~8周临床愈合,6~9个月骨性愈合(平均8个月),并取除内固定装置,均未发生明显肢体短缩、增长、旋转、感染、骨不连、畸形愈合、内固定断裂等并发症,髋、膝关节活动正常。结论:两种术式均疗效可靠,逆行髓内钉操作简便、创伤小、出血量少、手术时间短、并发症少,适用于大部分非粉碎性骨折及易复位的粉碎性骨折,钢板螺钉适用于复杂粉碎性骨折。

목적:탐토역행수내정여강판라정치료인동고골간골절적효과。방법:회고성분석51례인동고골간골절수술치료방법급결과。결과:역행수내정고정33례,강판라정고정18례,수방5개월~4년5개월(평균2년6개월),균재4~8주림상유합,6~9개월골성유합(평균8개월),병취제내고정장치,균미발생명현지체단축、증장、선전、감염、골불련、기형유합、내고정단렬등병발증,관、슬관절활동정상。결론:량충술식균료효가고,역행수내정조작간편、창상소、출혈량소、수술시간단、병발증소,괄용우대부분비분쇄성골절급역복위적분쇄성골절,강판라정괄용우복잡분쇄성골절。
Objective: To discuss the treating effects of femoral shaft fracture of children in various operations.Methods:The operative methods and results from 51 cases of femur shaft fractures of children were studied retrospectively.Results:Retrograde nail fixations were performed in 33 cases,plate and screw fixations were performed in 18 cases.Follow uPtime ranged from 5 months to 4 years and 5 months (averaged,2years and 6 months). All cases achieved clinical healings in 4 to 8 weeks after operations.Internal fixation devices were taken out in 6 to 9months (averaged,8 months) after bone union.No cases performed limb shortening,length or rotation infection bone nonunion,malunion,fixation fracture.The knee and hiPjoint activity are good. Conclusion:The two operations are all effective,Retrograde nail fixation is simple to be handled,has the surgery wound to be small,less bleeding,complications and the operation time.It is indicated for most non comminuted fracture and simple comminuted fracture.Plate fixation is indicated for complex comminuted fracture.
