目的:探讨增强CT、高分辨率CT( HRCT)及18 F-氟脱氧葡萄糖联合正电子发射计算机断层显像(18 F-FDG PET/CT)对早期周围型肺癌的诊断价值。方法回顾性分析2011年1月至2013年12月收治的临床疑似早期肺癌患者42例,分析增强CT、HRCT及18 F-FDG PET/CT检查结果。结果18 F-FDG PET/CT诊断早期周围型肺癌的灵敏度、特异度、阳性预测值、阴性预测值、Youden指数均明显高于增强CT和HRCT,差异有统计学意义( P <0.05)。周围型肺癌组和非周围型肺癌组平扫Ct值差异无统计学意义( P >0.05),而增强后Ct值和CT净增值周围型肺癌组明显高于非周围型肺癌组,差异有统计学意义( P <0.05)。且周围型肺癌组的标准化最大摄取量( SUVmax)水平也明显高于非周围型肺癌组,差异有统计学意义( P <0.05)。结论18 F-FDG PET/CT对早期周围型肺癌的诊断效能高于增强CT和HRCT,而增强CT中CT净增值在判断周围型肺癌时有实际意义,单纯HRCT诊断效能较低。

목적:탐토증강CT、고분변솔CT( HRCT)급18 F-불탈양포도당연합정전자발사계산궤단층현상(18 F-FDG PET/CT)대조기주위형폐암적진단개치。방법회고성분석2011년1월지2013년12월수치적림상의사조기폐암환자42례,분석증강CT、HRCT급18 F-FDG PET/CT검사결과。결과18 F-FDG PET/CT진단조기주위형폐암적령민도、특이도、양성예측치、음성예측치、Youden지수균명현고우증강CT화HRCT,차이유통계학의의( P <0.05)。주위형폐암조화비주위형폐암조평소Ct치차이무통계학의의( P >0.05),이증강후Ct치화CT정증치주위형폐암조명현고우비주위형폐암조,차이유통계학의의( P <0.05)。차주위형폐암조적표준화최대섭취량( SUVmax)수평야명현고우비주위형폐암조,차이유통계학의의( P <0.05)。결론18 F-FDG PET/CT대조기주위형폐암적진단효능고우증강CT화HRCT,이증강CT중CT정증치재판단주위형폐암시유실제의의,단순HRCT진단효능교저。
Objective To discuss the diagnostic value of the enhanced CT,HRCT and 18 F-FDG PET/CT in the early peripheral lung cancer. Methods Retrospective analysis of 42 patients with suspected early lung cancer in our hospital from January 2011 to December 2013 were performed. The analysis indexes included enhanced CT,HRCT,and 18 F-FDG PET/CT findings. Results The sensitivity of diagnosis of early peripheral lung cancer( SEN),specificity( SPE),the positive predictive value( PV+),the negative predictive value( PV-),the Youden index of 18 F-FDG PET/CT were higher than those of enhanced CT and HRCT. There was statistical significance in CT value after enhancement and CT net value difference( P <0. 05)Peripheral lung cancer group and non peripheral lung cancer group were compared between SUVmax,the difference was statistically significant( P <0. 05). Conclusion 18 F -FDG PET/CT diagnosis efficiency was higher than enhanced CT and HRCT. The CT value in the judgment of peripheral lung cancer has practical significance,diagnostic efficiency of HRCT is low.
