目的：探讨三角吻合术在胃癌患者腹腔镜下远端胃切除术中的应用。方法选择19例胃癌患者，在腹腔镜下远端胃切除术中采用三角吻合术，观察手术结果。结果患者总手术时间为（179.3±39.5）min，胃十二指肠三角吻合术所用时间为（21.2±8.9）min。术中每例患者平均应用直线形吻合器钉匣（5.2±1.0）个。术中总出血量约（53.8±12.9）ml。每例患者淋巴结清扫数目（33.4±10.7）枚。术后未出现吻合口瘘、吻合口狭窄及吻合口出血等并发症。结论胃十二指肠三角吻合术是胃癌全腹腔镜下远端胃切除术消化道重建的理想吻合技术，值得临床应用。
목적：탐토삼각문합술재위암환자복강경하원단위절제술중적응용。방법선택19례위암환자，재복강경하원단위절제술중채용삼각문합술，관찰수술결과。결과환자총수술시간위（179.3±39.5）min，위십이지장삼각문합술소용시간위（21.2±8.9）min。술중매례환자평균응용직선형문합기정갑（5.2±1.0）개。술중총출혈량약（53.8±12.9）ml。매례환자림파결청소수목（33.4±10.7）매。술후미출현문합구루、문합구협착급문합구출혈등병발증。결론위십이지장삼각문합술시위암전복강경하원단위절제술소화도중건적이상문합기술，치득림상응용。
Objective To explore the role of triangular anastomosis in laparoscopic distal gastrectomy in patients with gastric cancer. Methods A total of 19 cases of gastric cancer were collected in this study. Triangular anastomosis had been applied in laparoscopic distal gastrec-tomy and the results of operation had been observed. Results The total operating time was 179. 3 ± 39. 5 min. The time for performance of gas-troduodenal triangular anastomosis was 21. 2 ± 8. 9 min. In each patient,the total number of linear stapler magazines reached 5. 2 ± 1. 0. The total amount of intraoperative blood loss was 53. 8 ± 12. 9 ml. The total number of dissected lymph nodes in one patient reached 33. 4 ± 10. 7 pieces. There was no anastomotic leakage,anastomotic stricture,anastomotic bleeding and other complications occurred in them. Conclusion Triangular gastroduodenal anastomosis is an ideal procedure for reconstruction of gastrointestinal anastomosis in laparoscopic gastric distal gastrectomy,hence it is worthy to be recommended for clinical application.
