目的:检测支气管哮喘患者外周血中白细胞介素-21( IL-21)水平,分析其与哮喘患者外周血炎症细胞的关系,探索IL-21与支气管哮喘的相关性。方法选取16例支气管哮喘发作期患者和13例缓解期患者分别作为支气管哮喘发作组和缓解组,同时选取30名健康体检者作为健康对照组。采用ELISA法分别检测3个组外周血血清中IL-21含量,比较3个组IL-21水平;同时,检测支气管哮喘发作组外周血中中性粒细胞( NEU)、淋巴细胞( LYM)和嗜酸性粒细胞( EOS)数量,分析IL-21与炎症细胞数量的相关性。结果支气管哮喘发作组血清IL-21水平为(486.23±159.93) ng/L,显著高于缓解组(271.46±80.37 ng/L,P<0.05)和健康对照组(210.64±125.41 ng/L,P<0.05)。支气管哮喘发作期IL-21水平与NEU和LYM数量均呈明显正相关(r=0.758,P=0.001;r=0.645,P=0.007),而与EOS的数量相关性无显著性意义(r=0.332,P=0.209)。结论 IL-21可能参与了支气管哮喘的免疫炎症反应机制。

목적:검측지기관효천환자외주혈중백세포개소-21( IL-21)수평,분석기여효천환자외주혈염증세포적관계,탐색IL-21여지기관효천적상관성。방법선취16례지기관효천발작기환자화13례완해기환자분별작위지기관효천발작조화완해조,동시선취30명건강체검자작위건강대조조。채용ELISA법분별검측3개조외주혈혈청중IL-21함량,비교3개조IL-21수평;동시,검측지기관효천발작조외주혈중중성립세포( NEU)、림파세포( LYM)화기산성립세포( EOS)수량,분석IL-21여염증세포수량적상관성。결과지기관효천발작조혈청IL-21수평위(486.23±159.93) ng/L,현저고우완해조(271.46±80.37 ng/L,P<0.05)화건강대조조(210.64±125.41 ng/L,P<0.05)。지기관효천발작기IL-21수평여NEU화LYM수량균정명현정상관(r=0.758,P=0.001;r=0.645,P=0.007),이여EOS적수량상관성무현저성의의(r=0.332,P=0.209)。결론 IL-21가능삼여료지기관효천적면역염증반응궤제。
Objective To study the relationship between interleukin-21 ( IL-21) and bronchial asthma. Methods 16 cases with bronchial asthma at the acute phase were selected as the acute attack group, 13 bronchial asthma cases at the remission stage as the remission group.Meanwhile, 30 healthy people were selected as healthy control group.The serum IL-21 levels in the peripheral blood for the 3 groups were detected by Double-Antibody Sandwich ELISA, respectively.The number of neutrophil granulocyte ( NEU) , lymphocyte ( LYM) and eosinophil granulocyte ( EOS) in peripheral blood for the acute attack group were also analyzed on the correlation between IL-21 level and the number of inflammatory cells.Results The IL-21 serum level of bronchial asthma cases at the acute attack group was 486.23 ±159.93 ng/L, and significantly higher than those at the remission group (271.46 ±80.37 ng/L) and healthy control group (210.64 ±125.41 ng/L).The IL-21 serum level of the acute attack group showed significantly positive correlation with the number of NEU and LYM ( r=0.758, P=0.001;r=0.645, P=0.007), but showed a non-significant correlation with EOS (r=0.332, P=0.209). Conclusions IL-21, as an inflammatory factor, maybe participates in the immune-inflammatory response of bronchial asthma.
