目的:分析癌胚抗原(CEA)、糖类抗原125(CA125)、胃泌素释放肽前体(pro-GRP)、细胞角蛋白19片段(CYFRA21-1)、神经元特异性烯醇化酶(NSE)等肿瘤标志物(TM)在肺癌早期诊断及预后判断中的价值。方法治疗前、后检测102例肺癌组、84例良性肺病组及40例健康组血清CEA、CA125、pro-GRP、CYFRA21-1、NSE水平,计算这5种TM对肺癌诊断的敏感度、特异度。结果健康组、良性肺病组、肺癌组CEA、CA125、pro-GRP、CYFRA21-1、NSE阳性率及浓度均逐渐升高,肺癌组显著高于其他两组,差异有统计学意义( P <0.01);5种TM联合检测诊断肺癌的敏感度、特异度均高于单项检测;治疗有效患者CEA、CA125、pro-GRP、CYFRA21-1、NSE浓度均低于无效患者( P<0.01)。结论 CEA、CA125、pro-GRP、CYFRA21-1、NSE 5种TM联合检测可明显提高肺癌诊断的灵敏度、特异度,为肺癌的早期诊断、疗效检测和预后评估提供参考。

목적:분석암배항원(CEA)、당류항원125(CA125)、위비소석방태전체(pro-GRP)、세포각단백19편단(CYFRA21-1)、신경원특이성희순화매(NSE)등종류표지물(TM)재폐암조기진단급예후판단중적개치。방법치료전、후검측102례폐암조、84례량성폐병조급40례건강조혈청CEA、CA125、pro-GRP、CYFRA21-1、NSE수평,계산저5충TM대폐암진단적민감도、특이도。결과건강조、량성폐병조、폐암조CEA、CA125、pro-GRP、CYFRA21-1、NSE양성솔급농도균축점승고,폐암조현저고우기타량조,차이유통계학의의( P <0.01);5충TM연합검측진단폐암적민감도、특이도균고우단항검측;치료유효환자CEA、CA125、pro-GRP、CYFRA21-1、NSE농도균저우무효환자( P<0.01)。결론 CEA、CA125、pro-GRP、CYFRA21-1、NSE 5충TM연합검측가명현제고폐암진단적령민도、특이도,위폐암적조기진단、료효검측화예후평고제공삼고。
Objective To analyze the value of CEA,CA125,pro-GRP,CYFRA21-1,NSE in the early diagnosis and treatment of lung cancer. Methods To detect the level of CEA,CA125,pro-GRP,CYFRA21-1,NSE in 102 patients with lung cancer,84 patients with benign lung disease,and 40 healthy. We calculated the sensitivity,specificity in the diagnosis of lung cancer of these TM. Results The positive rate and concentration of CEA,CA125,pro-GRP,CYFRA21-1,NSE were gradually increased in healthy group,benign lung disease group,lung canc-er group,lung cancer group. There was significant difference( P <0. 01). The sensitivity,specificity of 5 kind TM joint detection in the diagno-sis of lung cancer were higher than the single test. The treatment was effective in patients with CEA,CA125,pro-GRP,CYFRA21 -1. The NSE concentrations were below the invalid patients( P <0. 01). Conclusion CEA,CA125,pro-GRP,CYFRA21-1,NSE 5 kinds TM joint detection can significantly improve the sensitivity and specificity of diagnosis of lung cancer,and provide a reference for the early diagnosis,effec-tive detection and prognosis of lung cancer.
