目的：观察术后应用右美托咪啶和丙泊酚治疗全麻后苏醒期躁动的疗效。方法：选择ASAⅠ或Ⅱ级全麻下行胃癌根治术且术毕出现躁动患者72例，随机分为M组和B组，每组36例。两组术后均用舒芬太尼总量2μg/kg行病人自控静脉镇痛，当苏醒期躁动评分≥2分时，M组静脉泵注右美托咪啶0.75μg/kg，B组静脉泵注丙泊酚1.5mg/kg。观察两组抗躁动起效时间和清醒时间、VAS评分、两组患者的Ramsay镇静评分及副作用。结果：M组和B组镇静深度均在4分以上，抗燥动疗效均达100%。B组比M组起效快，但差异无统计学意义（P＞0.05），两组清醒时间比较差异亦无统计学意义（P＞0.05）；与B组比较，拔管后M组VAS评分明显降低（P＜0.05）；M组有2例、B组有13例发生循环呼吸抑制，差异有统计学意义（P＜0.05）；两组患者均未发生恶心呕吐和肌颤。结论：右美托咪啶和丙泊酚均可有效的治疗全麻后苏醒期躁动，右美托咪啶对呼吸循环影响较轻，增加耐受性，更加安全。
목적：관찰술후응용우미탁미정화병박분치료전마후소성기조동적료효。방법：선택ASAⅠ혹Ⅱ급전마하행위암근치술차술필출현조동환자72례，수궤분위M조화B조，매조36례。량조술후균용서분태니총량2μg/kg행병인자공정맥진통，당소성기조동평분≥2분시，M조정맥빙주우미탁미정0.75μg/kg，B조정맥빙주병박분1.5mg/kg。관찰량조항조동기효시간화청성시간、VAS평분、량조환자적Ramsay진정평분급부작용。결과：M조화B조진정심도균재4분이상，항조동료효균체100%。B조비M조기효쾌，단차이무통계학의의（P＞0.05），량조청성시간비교차이역무통계학의의（P＞0.05）；여B조비교，발관후M조VAS평분명현강저（P＜0.05）；M조유2례、B조유13례발생순배호흡억제，차이유통계학의의（P＜0.05）；량조환자균미발생악심구토화기전。결론：우미탁미정화병박분균가유효적치료전마후소성기조동，우미탁미정대호흡순배영향교경，증가내수성，경가안전。

Results:M depth of sedation group and B group were at least 4 points,resistance to dry dynamic curative effect was 100%;GrouPB than in group M work fast,but there was no statistically significant difference (P>0.05),the waking hours more similar between the two groups has no statistical significance (P>0.05);Compared with group B,group M after extubation VAS scores significantly decreased (P<0.05);M group 2,group B patients have 13 cases had respiratory depression,the difference was statistically significant (P<0.05).Conclusion:Two groups of patients were not occurred nausea and vomiting and muscle tremors.conclusion right the pyrimidine and propofol can effective treatment after general anesthesia to stir,right the pyrimidine has less impact on the respiratory cycle,increase the tolerance,more secure.
