目的:探讨双源 CT 双能量扫描技术对正常甲状腺组织碘含量及增强后碘含量的应用价值,为甲状腺良恶性疾病的临床诊断提供依据。方法选取300例经超声检查排除甲状腺疾病的门诊病例,进行双源 CT 甲状腺扫描。将扫描数据传至工作站 MMWP 上的双能量软件中,选择“Liver VNC”模式得到碘基图像,测量正常甲状腺组织、胸锁乳突肌的碘含量,并计算二者碘含量比值,采用 t 检验进行统计学分析。结果女性甲状腺平扫碘含量为(1.41±0.22)mg/mL,增强碘含量为(4.55±1.32)mg/mL;男性甲状腺平扫碘含量为(1.35±0.19)mg/mL,增强碘含量为(5.23±1.41)mg/mL。甲状腺平扫与增强碘含量比较差异有统计学意义(t =40.4244,P =0.000)。甲状腺平扫与胸锁乳突肌碘含量比值为(5.71±2.43),女性比值为(5.65±2.63),男性比值为(6.09±2.57),男女性比值差异无统计学意义(t =-0.668,P =0.358)。甲状腺增强与胸锁乳突肌碘含量比值比为(6.86±4.57),女性比值为(6.77±3.55),男性比值为(7.03±3.82),男女性比值差异无统计学意义(t =-1.750,P =0.235)。甲状腺平扫左叶碘含量为(1.49±0.19)mg/mL,右叶碘含量为(1.42±0.17)mg/mL,左、右叶碘含量比较差异无统计学意义(t =-0.153,P =0.754)。甲状腺增强左叶碘含量为(4.89±0.22)mg/mL,右叶碘含量为(5.42±0.33)mg/mL,左、右叶碘含量比较差异无统计学意义(t =-0.323,P =0.645)。结论双源 CT 双能量扫描技术能够准确测量正常甲状腺组织碘含量,为临床对诊断状腺疾病提供影像学依据。

Objective To measure the iodine content of the normal thyroid tissue before and after enhance-ment by using dual-energy scanning technique,and provide basis for clinical diagnosis of thyroid disease. Methods Selected 300 patients from the Second Affiliated Hospital of Xinjiang Medical University.All of them are the normal group who were examed to exclude the thyroid disease by ultrasound.All patients in-cluded in the study are asked to make dual-source CT examination of the neck.Transform the CT data of the normal group to the workstation MMWP of the Dual-Energy Software.Select “Liver VNC”mode and get iodine distribution map.Measuring the normal group′s thyroid iodine content per unit volume during plain scanning and after the enhanced.The data was analyzed by t test,and P<0.05 indicated the differ-ence wasstatistically significant.Results The females′thyroid iodine content per unit volume is (1.41 ± 0.22)mg/mL during plain scanning and the males′ is (1.35 ± 0.19)mg/mL.The females′ thyroid iodine content per unit volume is (4.55± 1.32)mg/mL in arterial phase enhancement and the males′is (5.23± 1.41)mg/mL.There is a significant difference between plain scanning and the arterial phase enhancement (t =40.4244,P =0.000).The ratio of thyroid iodine content per unit volume and sternocleidomastoid is (5.71±2.43),Where females′ratio is (5.65±2.63),the ratio of males is (6.09±2.57)and there is no sig-nificant difference between them (t =-0.668,P =0.358).The ratio of thyroid iodine content per unit vol-ume and sternocleidomastoid in arterial phase enhancement is 6.86±4.57,Where females′ratio is (6.77± 3.55),the ratio of males is (7.03±3.82)and there is no significant difference between them (t =-1.750, P =0.235).The unit volume iodine of the left lobe of the thyroid is (1.49±0.19)mg/mL and the right is (1.42±0.17)mg/mL during the plain scanning of normal group′s Thyroid.The difference between left and right lobe is no statistically significant (t =-0.153,P =0.754).The unit volume iodine of the left lobe of the thyroid is (4.89±0.22)mg/mL and the right is (5.42 ±0.33)mg/mL during the arterial phase en-hancement of thyroid in normal group.The difference between left and right lobe is no statistically signifi-cant (t =-0.323,P =0.645).Conclusion We can measure the iodine content of normal thyroid tissue dur-ing plain scanning and enhancement by using dual-source CT dual-energy scanning technology,Provide the basis for clinical diagnosis of thyroid disease.
