目的：分析内镜下切除联合皮圈结扎闭合术治疗胃固有肌层肿瘤的疗效。方法选择接受住院治疗的胃固有肌层肿瘤患者作为研究对象，随机分为接受内镜下黏膜结扎剥离术治疗的对照组、接受内镜下切除联合皮圈结扎闭合术治疗的观察组，比较两组患者的手术相关指标、术中并发症及随访复发率、应激指标（包括肾上腺素、去甲肾上腺素以及血管紧张素Ⅱ）水平等差异。结果①观察组切除标本直径、手术操作时间、平均住院时间等均明显小于对照组患者（ P ﹤0.05）。②观察组胃出血、气腹、局限性腹膜炎等术后并发症发生率均明显低于对照组（ P ﹤0.05），随访1年及3年的肿瘤复发率明显低于对照组（ P ﹤0.05）。③观察组术中各项应激指标水平均明显低于对照组（ P ﹤0.05）。结论内镜下切除联合皮圈结扎闭合术可以完整切除胃固有肌层肿瘤，同时有效减少胃穿孔带来的一系列并发症，稳定围术期患者的应激水平。
목적：분석내경하절제연합피권결찰폐합술치료위고유기층종류적료효。방법선택접수주원치료적위고유기층종류환자작위연구대상，수궤분위접수내경하점막결찰박리술치료적대조조、접수내경하절제연합피권결찰폐합술치료적관찰조，비교량조환자적수술상관지표、술중병발증급수방복발솔、응격지표（포괄신상선소、거갑신상선소이급혈관긴장소Ⅱ）수평등차이。결과①관찰조절제표본직경、수술조작시간、평균주원시간등균명현소우대조조환자（ P ﹤0.05）。②관찰조위출혈、기복、국한성복막염등술후병발증발생솔균명현저우대조조（ P ﹤0.05），수방1년급3년적종류복발솔명현저우대조조（ P ﹤0.05）。③관찰조술중각항응격지표수평균명현저우대조조（ P ﹤0.05）。결론내경하절제연합피권결찰폐합술가이완정절제위고유기층종류，동시유효감소위천공대래적일계렬병발증，은정위술기환자적응격수평。
Objectine To analyze the efficacy of endoscopic resection combined with band ligation closure in treatment of patients with gastric submucosal tumor. Methods Patients with gastric submucosal tumor in this hospital were allocated in this study,they were divided into control group and observation group,patients in control group received endoscopic mucosa ligation and stripping,patients in observation group re-ceived endoscopic band ligation closure,and indicators related to operation,intraoperative complications and recurrence rate in follow - up period and stress level of indicators were compared between these 2 groups. Results The diameter of specimens,operating time,average hospital stay in observation group were significantly less than those of control group( P ﹤ 0. 05). The incidence rates of stomach bleeding,pneumoperitoneum, localized peritonitis etc postoperative complications in observation group were significantly lower than those of control group. The recurrence rates in follow - up periods for 1 year and 3 years were significantly lower than those of control group( P ﹤ 0. 05). The various indicators of stress level in observation group were significantly lower than those of control group( P ﹤ 0. 05). Conclusion Endoscopic resection combined skin closed ring ligation can completely resect the gastric tumor in muscularis propria,and effectively reduce the complications caused by gastric perforation and keep the stability of stress level. of patients during perioperative period.
