目的：探讨1，25-二羟维生素 D3辅助布地奈德对支气管哮喘儿童血清中基质金属蛋白酶9（MMP -9）和白细胞介素10（IL -10）的影响。方法102例支气管哮喘患儿随机分为两组，分别给予单纯布地奈德（布地奈德组， n ＝51）和布地奈德联合1，25-二羟维生素 D3（布地奈德加1，25-二羟维生素 D3组，n ＝51）两种方法进行治疗。另选同期51例健康体检儿童为对照组。比较两治疗组临床疗效及三组儿童血清 MMP -9和 IL -10水平。结果布地奈德加1，25-二羟维生素 D3组有效率明显高于布地奈德组，差异有统计学意义（ P ﹤0.05）。两治疗组 IL -10水平均明显低于对照组（ P ﹤0.05），MMP -9水平明显高于对照组（ P ﹤0.05）。结论1，25-二羟维生素 D3联合布地奈德治疗哮喘较单纯布地奈德更为有效；1，25-二羟维生素 D3联合布地奈德更有效地提高了布地奈德促 IL -10分泌的能力，降低 MMP -9的活性；此方法从抗炎和改善气道重塑两个方面有效地治疗了儿童支气管哮喘。
목적：탐토1，25-이간유생소 D3보조포지내덕대지기관효천인동혈청중기질금속단백매9（MMP -9）화백세포개소10（IL -10）적영향。방법102례지기관효천환인수궤분위량조，분별급여단순포지내덕（포지내덕조， n ＝51）화포지내덕연합1，25-이간유생소 D3（포지내덕가1，25-이간유생소 D3조，n ＝51）량충방법진행치료。령선동기51례건강체검인동위대조조。비교량치료조림상료효급삼조인동혈청 MMP -9화 IL -10수평。결과포지내덕가1，25-이간유생소 D3조유효솔명현고우포지내덕조，차이유통계학의의（ P ﹤0.05）。량치료조 IL -10수평균명현저우대조조（ P ﹤0.05），MMP -9수평명현고우대조조（ P ﹤0.05）。결론1，25-이간유생소 D3연합포지내덕치료효천교단순포지내덕경위유효；1，25-이간유생소 D3연합포지내덕경유효지제고료포지내덕촉 IL -10분비적능력，강저 MMP -9적활성；차방법종항염화개선기도중소량개방면유효지치료료인동지기관효천。
Objectine To evaluate the effect of budesonide combined with 1,25 -(OH)2 D3 on the expressions of MMP - 9 and IL - 10 in children with asthma. Methods One hundred and two children with asthma were randomly divided into two groups:the budesonide group,the budesonide combined with 1,25 -(OH)2 D3 group. Fifty - one healthy children were in the control group. The levels of serum IL - 10 and MMP- 9 were measured by enzyme - linked immunosorbent assay(ELISA). Results The effective rate in budesonide combined with 1,25 -(OH)2 D3 was significantly higher than budesonide group( P ﹤ 0. 05). Compared with the budesonide combined with 1,25 -(OH)2 D3 group,the level of IL - 10 in budesonide group was lower( P ﹤ 0. 05),and the level of MMP - 9 was higher( P ﹤ 0. 05). Conclusion The study indicates that 1,25 -(OH)2 D3 combined with budesonide has better clinical efficacy to treat asthma. 1,25 -(OH)2 D3 combined with budesonide can en-hance the activity of IL - 10 and down - regulate the expression of MMP - 9 in the serum of children with asthma,thus alleviating the inflammatory responses and improving the airway remodeling and therefore treat the children with asthma.
