目的:探讨1,25-二羟维生素 D3辅助布地奈德对支气管哮喘儿童血清中基质金属蛋白酶9(MMP -9)和白细胞介素10(IL -10)的影响。方法102例支气管哮喘患儿随机分为两组,分别给予单纯布地奈德(布地奈德组, n =51)和布地奈德联合1,25-二羟维生素 D3(布地奈德加1,25-二羟维生素 D3组,n =51)两种方法进行治疗。另选同期51例健康体检儿童为对照组。比较两治疗组临床疗效及三组儿童血清 MMP -9和 IL -10水平。结果布地奈德加1,25-二羟维生素 D3组有效率明显高于布地奈德组,差异有统计学意义( P ﹤0.05)。两治疗组 IL -10水平均明显低于对照组( P ﹤0.05),MMP -9水平明显高于对照组( P ﹤0.05)。结论1,25-二羟维生素 D3联合布地奈德治疗哮喘较单纯布地奈德更为有效;1,25-二羟维生素 D3联合布地奈德更有效地提高了布地奈德促 IL -10分泌的能力,降低 MMP -9的活性;此方法从抗炎和改善气道重塑两个方面有效地治疗了儿童支气管哮喘。

Objectine To evaluate the effect of budesonide combined with 1,25 -(OH)2 D3 on the expressions of MMP - 9 and IL - 10 in children with asthma. Methods One hundred and two children with asthma were randomly divided into two groups:the budesonide group,the budesonide combined with 1,25 -(OH)2 D3 group. Fifty - one healthy children were in the control group. The levels of serum IL - 10 and MMP- 9 were measured by enzyme - linked immunosorbent assay(ELISA). Results The effective rate in budesonide combined with 1,25 -(OH)2 D3 was significantly higher than budesonide group( P ﹤ 0. 05). Compared with the budesonide combined with 1,25 -(OH)2 D3 group,the level of IL - 10 in budesonide group was lower( P ﹤ 0. 05),and the level of MMP - 9 was higher( P ﹤ 0. 05). Conclusion The study indicates that 1,25 -(OH)2 D3 combined with budesonide has better clinical efficacy to treat asthma. 1,25 -(OH)2 D3 combined with budesonide can en-hance the activity of IL - 10 and down - regulate the expression of MMP - 9 in the serum of children with asthma,thus alleviating the inflammatory responses and improving the airway remodeling and therefore treat the children with asthma.
