目的:探讨曲美他嗪与尼可地尔对经皮冠状动脉介入( PCI)治疗相关心肌损伤干预作用。方法将行PCI的不稳定型心绞痛患者70例随机分为对照组、曲美他嗪组和尼可地尔组,分别采用不同药物治疗,比较心肌损伤相关指标水平。结果术前3组CK-MB、IcTnI水平差异无统计学意义(P>0.05);3组术后6、12、24小时CK-MB与cTnI水平显著升高,差异有统计学意义(P<0.05);曲美他嗪组与尼可地尔组患者术后MPV水平降低,差异有统计学意义(P<0.05);曲美他嗪组与尼可地尔组术后CK-MB、cTnI水平较对照组显著降低,差异有统计学意义( P<0.05),但曲美他嗪组与尼可地尔组患者术后CK-MB与cTnI水平进行比较,差异无统计学意义(P>0.05)。结论 PCI术前使用曲美他嗪与尼可地尔,可减少PCI引起的心肌损伤,发挥心肌保护的作用。

목적:탐토곡미타진여니가지이대경피관상동맥개입( PCI)치료상관심기손상간예작용。방법장행PCI적불은정형심교통환자70례수궤분위대조조、곡미타진조화니가지이조,분별채용불동약물치료,비교심기손상상관지표수평。결과술전3조CK-MB、IcTnI수평차이무통계학의의(P>0.05);3조술후6、12、24소시CK-MB여cTnI수평현저승고,차이유통계학의의(P<0.05);곡미타진조여니가지이조환자술후MPV수평강저,차이유통계학의의(P<0.05);곡미타진조여니가지이조술후CK-MB、cTnI수평교대조조현저강저,차이유통계학의의( P<0.05),단곡미타진조여니가지이조환자술후CK-MB여cTnI수평진행비교,차이무통계학의의(P>0.05)。결론 PCI술전사용곡미타진여니가지이,가감소PCI인기적심기손상,발휘심기보호적작용。
Objective To explore the myocardial damage intervention effect of trimetazidine and nicole on PCI treatment. Methods Seventy cases of patients with unstable angina treated in this hospital were randomly divided into control group, trimetazidine group and nicole group. Different treatment was applied, and the level of creatine kinase isoenzyme ( CK-MB) , troponin I ( cTnI) and average blood platelet volume ( MPV) of the groups before surgery, 6 hours, 12 hours and 24 hours after surgery were compared respectively. Results Preoperative CK-MB, IcTnI level of the three groups had no statistical significance (P>0. 05). CK-MB and cTnI levels of the three groups were significantly elevated 6 h, 12 h, 24 h postoperatively(P<0. 05). Postoperative MPV levels at each time point in trimetazidine group and nicole group had significant difference (P<0. 05). Postoperative CK-MB, cTnI levels in trimetazidine group and nicole group were sig-nificantly lower than those in control group ( P<0. 05 ) , but there was no significant difference when the two groups were compared ( P>0. 05). Conclusion Treatment of the patients with trimetazidine or nicole before PCI surgery can reduce the myocardial injury caused by PCI surgery, thus it plays the role of myocardial protection.
