目的 探讨对慢性阻塞性肺疾病急性发作期( AECOPD)患者应用多索茶碱后患者血清IL-8、TNF-α、IL-10变化情况. 方法 将120例AECOPD患者随机分为观察组和对照组,观察组应用多索茶碱,对照组应用氨茶碱,比较两组治疗前后肺功能及血清IL-8、TNF-α、IL-10. 结果 ①治疗后,两组PaO2、FEV1、FVC、FEV1/FVC升高,PaCO2 降低(P<0. 05);且观察组改善更明显(P<0. 05);②治疗后,两组IL-10升高, IL-8、TNF-α降低,且观察组改善更明显(P<0. 05);③AECOPD患者FEV1/FVC与IL-8、IL-10成负相关,与IL-10成正相关( P<0. 05 ). 结论 血清IL-8、TNF-α、IL-10与AECOPD患者气道功能具有相关性,多索茶碱能有效的抑制IL-8、TNF-α水平,提升IL-10水平.

목적 탐토대만성조새성폐질병급성발작기( AECOPD)환자응용다색다감후환자혈청IL-8、TNF-α、IL-10변화정황. 방법 장120례AECOPD환자수궤분위관찰조화대조조,관찰조응용다색다감,대조조응용안다감,비교량조치료전후폐공능급혈청IL-8、TNF-α、IL-10. 결과 ①치료후,량조PaO2、FEV1、FVC、FEV1/FVC승고,PaCO2 강저(P<0. 05);차관찰조개선경명현(P<0. 05);②치료후,량조IL-10승고, IL-8、TNF-α강저,차관찰조개선경명현(P<0. 05);③AECOPD환자FEV1/FVC여IL-8、IL-10성부상관,여IL-10성정상관( P<0. 05 ). 결론 혈청IL-8、TNF-α、IL-10여AECOPD환자기도공능구유상관성,다색다감능유효적억제IL-8、TNF-α수평,제승IL-10수평.
Objective To study the effect of doxofylline on serum IL-8, TNF-α and IL-10 in patients with AECOPD. Methods 120 cases of AECOPD were randomly divided into the observation group and the control group. The observation group was treated with doxofylline, and the control group was treated with aminophylline. Their pul-monary function before and after treatment and the levels of serum IL-8, TNF-αand IL-10 were compared between the two groups. Results After the treatment, the levels of PaO2 , FEV1 , FVC and FEV1/FVC increased, and the level of PaCO2 decreased in the two groups ( P<0. 05 ) , but the improvement was more pronounced in the observation group than in the control group (P<0. 05). After the treatment, the level of IL-10 increased, and the levels of IL-8 and TNF-a decreased in the two groups, but it was more obviously in the observation group than in the control group (P<0. 05). The level of FEV1/FVC had negative correlation with IL-8 and IL-10, and positive correlation with IL-10 (P<0. 05). Conclusion There is correlation between the levels of serum IL-8, TNF-α and IL-10 and lung function in patients with AECOPD, and doxofylline can effectively inhibit IL-8 and TNF-α, and elevate IL-10.
