目的 研究血D-二聚体水平与社区获得性肺炎( CAP)患者死亡率的相关性,以探讨其在CAP患者死亡中的临床价值. 方法 将我院治疗的77例CAP患者根据入院时病情分为普通CAP组和高危CAP组,住院后根据存活情况分为存活组和死亡组,分别检测各组患者的血CRP和D-二聚体水平,并进行相关性分析研究. 结果 高危CAP组患者的血CRP和D-二聚体水平分别为(1043. 50 ± 148. 24)μg/L、(83. 25 ± 21. 14)μg/L,均显著高于普通CAP组患者,P<0. 05;存活组患者的血CRP和D-二聚体水平(574. 31 ± 68. 45)μg/L、(63. 48 ± 17. 32)μg/L,死亡组患者的血CRP水平(1358. 74 ± 242. 49)μg/L、(102. 53 ± 24. 72)μg/L,差异具有显著性,P<0. 05. 结论 血D-二聚体与CAP患者死亡密切相关,可作为评价CAP死亡风险的临床指标.

Objective To study the correlation between blood D-dimer level and mortality of community-ac-quired pneumonia ( CAP) , in order to explore its clinical value in patients with CAP. Methods 77 CAP patients were divided into the high-risk group and the low-risk group, and then they were divided into the survival group and the death group based on their prognosis after admission. Their blood CRP and D-dimer levels and correlation were analyzed. Results The levels of serum CRP and D-dimer were (1043. 50 ± 148. 24) μg/L and (83. 25 ± 21. 14)μg/L in the high-risk group, which were significantly higher than those of the low-risk group (P<0. 05). The levels of serum CRP and D-dimer were (574. 31 ± 68. 45) μg/L and (63. 48 ± 17. 32) μg/L in the survival group, and (1358. 74 ± 242. 49) μg/L and (102. 53 ± 24. 72) μg/L in the death group (P<0. 05). Conclusion The level of blood D-dimer is closely related with the death of patients with CAP, which can be used as a clinical evaluation of CAP index.
