目的 对比机械振动排痰与手工拍背排痰方法对尘肺患者全肺灌洗术后治疗效果. 方法 选取60例全肺灌洗术后的尘肺患者,随机分为两组. 对照组采用手工拍背排痰,实验组采用机械振动排痰. 观察两组患者5天后咳痰消失时间、动脉血气及肺功能指标. 结果 尘肺患者行肺灌洗术后,实验组和对照组患者咳痰平均消失时间为:3. 81 ± 1. 22天和4. 32 ± 1. 71天( P<0. 05 ). 实验组排痰治疗后PaO2 和SaO2 上升值(14. 33 ± 3. 51 mmHg 和5. 63 ± 2. 42%)明显高于对照组( 8. 12 ± 2. 3 mmHg 和3. 01 ± 2. 02%) ( P <0. 05). 实验组排痰后FEV1 平均上升值(1. 01 ±0. 43L)明显高于对照组(0. 62 ±0. 52L)(P<0. 05). 结论应用机械振动排痰仪对全肺灌洗术后尘肺患者排痰,效果优于手工拍背排痰.

Objective To compare the curative effect between mechanical vibration sputum evacuation and manual sputum evacuation in the treatment of pneumoconiosis patients after whole lung lavage. Methods 60 patients after whole lung lavage were randomly divided into 2 groups. The control group was given manual sputum evacuation, and the experimental group was given mechanical vibration sputum evacuation. Their sputum disappear time, arterial blood gas and lung function indexes were observed 5 days after whole lung lavage. Results After lung lavage, the sputum disappear time were 3. 81 ± 1. 22 days and 4. 32 ± 1. 71 days respectively in the experimental group and the control group (P<0. 05). The elevated value of SaO2 and PaO2 in the experimental group after the treatment of spu-tum evacuation was significantly higher than that of the control group ( P <0. 05 ) . The average elevated value of FEV1 in the experimental group was significantly higher than that in the control group ( P<0. 05 ) . Conclusion The application of mechanical vibration sputum evacuation has better curative effect than manual sputum evacuation does in the treatment of pneumoconiosis patients after whole lung lavage.
