目的 了解我院近两年就诊患者分枝杆菌对10种药物的耐药状况,为临床化疗方案的选择提供科学依据. 方法 将收集到的痰标本进行分离培养,然后利用噻吩-2-羧酸肼( TCH)培养基生长试验、硝酸还原试验以及烟酸试验对其进行菌种鉴定,并同时采用绝对浓度法对10种抗结核药物进行药敏试验. 结果结核分枝杆菌(包括人型和牛型)共377株,对4种一线抗结核药的总耐药率为15. 11%( 57/377 ) ,耐多药结核(MDR-TB)有3株(0. 8%). 非结核分枝杆菌128 株,对4 种一线抗结核药的总耐药率为66. 9%(97/128),对10种药物全耐药的有22株(17. 1%). 结论 分枝杆菌(尤其是非结核分枝杆菌)对常用抗结核药物的耐药率高,应加强抗结核药物的耐药性监测,并根据药敏结果选择科学有效的化疗方案.

Objective To understand the drug resistance of mycobacteria to 10 drugs in our hospital in re-cent two years, in order to provide scientific basis for clinical choice of chemotherapy. Methods Bacteria were iso-lated and cultured from the sputum samples, then they were identified by PNB and TCH, and at the same time they were tested with absolute concentration method for 10 kinds of anti-tuberculosis drugs. Results There were 377 strains of tuberculosis mycobacteria, including human type and bovine type, which showed 15. 11% resistance to 4 kinds of first-line drugs ( 57/377 ) , and 3 strains of them were multi-drug resistant ( MDR-TB ) ( 0. 8%) . There were 128 strains of non-tuberculosis mycobacteria, and the total resistance rate for 4 first-line drugs was 66. 9% (97/128), including 22 strains resistant to all 10 types of drugs. Conclusion Mycobacteria, especially non-tuberculosis mycobacteria, show a high resistance to many drugs, so the monitoring of drug resistance to anti-tuberculosis drugs should be strengthened, and the reasonable and efficient chemotherapy plans should be chosen according to the drug sensitive test results.
