目的：研究2型糖尿病（diabetes mellitus，DM）合并胃癌（gastric cancer，GC）患者血清胰岛素样生长因子-1（insulin - like growth factor -1，IGF -1）、胰岛素样生长因子结合蛋白-3（insulin - like growth factor binding protein -3，IGFBP -3）的变化。方法：选择2型 DM 患者30例（DM 组）、2型 DM 合并 GC 患者48例（DM 并 GC 组）为研究对象，ELISA 法测定血清 IGF -1、IGFBP -3水平；同时测定空腹血糖、空腹胰岛素、血脂水平。结果：DM 并 GC 组的 IGF -1、IGF -1/ IGFBP -3比值高于 DM 组（P ＜0.05），IGFBP -3在两组之间无统计学差异（P ＞0.05）。IGF -1、IGF -1/ IGFBP -3比值与空腹胰岛素水平呈正相关（r =0.598，0.626， P =0.000），IGF -1/ IGFBP -3比值是 DM 并 GC 患者淋巴结转移的危险因素（OR =2.142，P =0.001）。结论：IGF -1可能参与2型 DM 合并 GC 的发生及淋巴结转移。
목적：연구2형당뇨병（diabetes mellitus，DM）합병위암（gastric cancer，GC）환자혈청이도소양생장인자-1（insulin - like growth factor -1，IGF -1）、이도소양생장인자결합단백-3（insulin - like growth factor binding protein -3，IGFBP -3）적변화。방법：선택2형 DM 환자30례（DM 조）、2형 DM 합병 GC 환자48례（DM 병 GC 조）위연구대상，ELISA 법측정혈청 IGF -1、IGFBP -3수평；동시측정공복혈당、공복이도소、혈지수평。결과：DM 병 GC 조적 IGF -1、IGF -1/ IGFBP -3비치고우 DM 조（P ＜0.05），IGFBP -3재량조지간무통계학차이（P ＞0.05）。IGF -1、IGF -1/ IGFBP -3비치여공복이도소수평정정상관（r =0.598，0.626， P =0.000），IGF -1/ IGFBP -3비치시 DM 병 GC 환자림파결전이적위험인소（OR =2.142，P =0.001）。결론：IGF -1가능삼여2형 DM 합병 GC 적발생급림파결전이。
Objective:To investigate changes of serum insulin - like growth factor - 1(IGF - 1)and insulin - like growth factor binding protein - 3( IGFBP - 3 )in patients with type 2 diabetes mellitus(DM)with gastric cancer (GC). Methods:Serum samples were collected from 30 DM patients(DM group),48 type 2 DM with GC patients (DM with GC group). ELISA was used to detect the levels of serum IGF - 1 and IGFBP - 3. Fasting plasma glucose, fasting insulin and lipids levels were measured. Results:There were higher levels of serum IGF - 1 and IGF - 1 / IG-FBP - 3 ratio in the DM with GC group than those in the DM group. There was no significant difference of serum IG-FBP - 3 in the two groups(P > 0. 05). IGF - 1 and IGF - 1 / IGFBP - 3 were positively correlated with fasting insulin (r = 0. 598,0. 626,P = 0. 000). IGF - 1 / IGFBP - 3 ratio was a risk factor of lymph node metastasis of DM with GC (OR = 2. 142,P = 0. 001). Conclusion:IGF - 1 may be involved in the development of DM with GC and lymph node metastasis.
