目的：观察头孢呋肟联合痰热清治疗老年慢性支气管炎的效果及其血气指标、炎性因子的变化情况。方法选择2013年4月—2014年6月老年慢性支气管炎患者184例，按照入院顺序将患者分为观察组（92例）和对照组（92例）。患者均予头孢呋肟治疗，观察组患者在此基础上使用痰热清注射液。比较2组患者治疗后的临床疗效及肺部啰音、呼吸困难、咳嗽改善时间，分析2组患者治疗前后的血气指标和炎性因子的变化情况。结果观察组患者总有效率为93‰．48％，高于对照组的80．43％（ P ＜0．05）；观察组患者的咳嗽改善时间为（3．1±0．4）d，呼吸困难改善时间为（3．2±0．8）d，肺部湿啰音改善时间为（2．8±0．6）d，短于对照组的（4．8±1．8）d、（5．1±1．9）d、（4．9±2．3）d（ P ＜0．05）；与治疗前比较，2组治疗后PaCO2、PaO2、CaO2以及血清CRP、IL-6、IL-8均改善（ P ＜0．01），且观察组治疗后优于对照组（ P ＜0．05）。结论慢性支气管炎老年患者应用头孢呋肟联合痰热清治疗，血气指标和炎性因子水平可以得到明显的改善，疗效较好，值得广泛推广使用。
목적：관찰두포부우연합담열청치료노년만성지기관염적효과급기혈기지표、염성인자적변화정황。방법선택2013년4월—2014년6월노년만성지기관염환자184례，안조입원순서장환자분위관찰조（92례）화대조조（92례）。환자균여두포부우치료，관찰조환자재차기출상사용담열청주사액。비교2조환자치료후적림상료효급폐부라음、호흡곤난、해수개선시간，분석2조환자치료전후적혈기지표화염성인자적변화정황。결과관찰조환자총유효솔위93‰．48％，고우대조조적80．43％（ P ＜0．05）；관찰조환자적해수개선시간위（3．1±0．4）d，호흡곤난개선시간위（3．2±0．8）d，폐부습라음개선시간위（2．8±0．6）d，단우대조조적（4．8±1．8）d、（5．1±1．9）d、（4．9±2．3）d（ P ＜0．05）；여치료전비교，2조치료후PaCO2、PaO2、CaO2이급혈청CRP、IL-6、IL-8균개선（ P ＜0．01），차관찰조치료후우우대조조（ P ＜0．05）。결론만성지기관염노년환자응용두포부우연합담열청치료，혈기지표화염성인자수평가이득도명현적개선，료효교호，치득엄범추엄사용。
Objective To study the effects of cefuroxime combined Tanreqing in treatment of elderly patients with chronic bronchitis and its effect on blood gas and inflammatory factors .Methods From April 2013 to June 2014 ,184 cases of elderly patients with chronic bronchitis , according to the order of admission patients were divided into observation group ( 92 cases) and control group (92 cases).All patients were given cefuroxime treatment , and the observation group patients were given Tanreqing on the basis of cefuroxime .Compared the clinical efficacy of the two groups of patients after treatment .Pulmo-nary rales improvement time and improvement of dyspnea time , as well as improvement of cough time were compared after treatment.Changes in blood gas and inflammatory factors were analyzed before and after treatment in the two groups of pa -tients.Results The overall effective rate in observation group (93.48%) was significantly higher than the control group (80.43%), the difference was statistically significant ( P <0.05).The patient's cough improve time (3.1 ±0.4) d, dysp-nea improve time (3.2 ±0.8) d and improve lung auscultation time (2.8 ±0.6) d obviously shorter than the control group , which were (4.8 ±1.8)d, (5.1 ±1.9)d and(4.9 ±2.3) d respectively,the difference was a statistically significance ( P <0.05).Compared with those before treatment, PaCO2, PaO2, CaO2 and serum CRP,IL-6,IL-8 in both groups were improved significantly,treatment group improved better than the control group ( P <0.05).Conclusion Apply cefuroxime and Tanreqing treatment on the elderly patients with chronic bronchitis , which can significantly improve blood gas indicators and inflammatory factors levels , the efficacy is good , which should be popularized use .
