目的:为降低医院药房退药率和提高医疗资源的使用效率提供参考。方法分析2014年1月至8月我国医疗机构退药原因文献的发表分布情况,并统计分析文献报道的退药原因记录。结果共检索出113篇文章,符合纳入标准的文献37篇,其中以《临床合理用药杂志》刊载量最高,占纳入文献的21.62%;以华东地区发表量最多,占纳入文献的45.95%。总退药记录数为133463条,其中以药品因素引起的最多,占37.32%;其次为患者因素、医务工作者与医院信息系统因素,分别占33.48%和27.00%。药品不良反应导致的退药占比最高,分别占药品因素退药和总退药记录数的47.92%和17.87%。结论退药情况分布在我国各地区,浪费了较多医疗资源。需提高医疗水平,加强合理用药和药品不良反应监测,降低退药率,以提高医疗资源利用率。

Objective To provide reference for decreasing drug return (DR) rate in hospitals and elevate the efficiency of medical re-sources. Methods Papers published from Jan. and Aug. 2014 were retrieved and analyzed for the publication distribution and the re-ported reasons of DR. Results 113 papers were retrieved,37 of which were included according to the including rules. The numbers of the papers published in Chinese Journal of Clinic Rational Drug Use,as well as East China area,were the most,accounting for 21. 62%and 45. 95% in included articles respectively. In a variety of causes of DR,the percentage related with drug was the most,accounting for 37. 32% in 133 463 records;the percentage of DR records caused by patients as well as medical worker and hospital information system factors accounted for 33. 48% and 27. 00% respectively;adverse drug reactions leading to the DR accounted for the highest proportion,47. 92% and 17. 87% for drug factors induced- and total drug return recorded number. Conclusion DR cases were extensively distributed in China,wasting many medical resources. Therefore,in order to reduce DR rate and improve the utilization of medical resources,it is necessary to improve the medical levels and strengthen the rational drug use,as well as the minitoring of adverse drug reactions.
