目的:探讨血乳酸、降钙素原(PCT)及C反应蛋白(CRP)在肺炎患者中的应用价值。方法整群选取2013年6月-2014年6月应用免疫荧光分析法测定85例老年肺部感染(肺炎组)患者及80例健康体检者(对照组)血清PCT水平,应用全自动化生化分析仪测定两组血乳酸,应用散射比浊法测定两组血清CRP。结果观察组血乳酸、PCT、CRP水平显著高于对照组,差异有统计学意义(P<0.05)。与存活组相比,死亡组血乳酸、PCT、CRP水平较高,差异有统计学意义(P<0.05)。经受试者特异曲线(ROC)分析显示,血乳酸、PCT、CRP联合检测时灵敏性、特异性、阳性预测值、阴性预测值高于各单项检测(P<0.05)。结论血乳酸、PCT、CRP水平可作为老年肺炎患者病情预后的评价指标。通过联合检测可提高血乳酸、PCT、CRP的在老年肺炎中的应用价值。

Objective To investigate application value of blood lactate (BLA), procalcitionin (PCT) and C-reactive protein (CRP) in elderly pneumonia. Methods Between June 2013 and June 2014, 85 elderly patients with pulmonary infection (observation group) and 80 healthy patients (control group) underwent determination of the levels of serum PCT, BLA and CRP using immunofluores-cence assay, automatic biochemical analyzer and scattering turbidimetry respectively. Results The levels of serum PCT, BLA and CRP were higher in the observation group than in the control group with statistically significant differences, P<0.05. Compared with those treated successfully, the levels of serum PCT, BLA and CRP were higher in the patients who died at last, and the dif-ferences were statistically significant, P<0.05. ROC analysis showed that combined detection of PCT, BLA and CRP can provide higher sensitivity, specificity, positive predictive value, and negative predictive value than individual detection, P<0.05. Conclu-sion Combined detection of PCT, BLA and CRP has higher application value in elderly pneumonia, while can indicate their prog-nosis.
